Description:

POSITION SUMMARY

The Adoray Home Health and Hospice Licensed Practical Nurse assists the Registered Nurse in performing specialized procedures, prepares equipment and materials for treatments, observing aseptic technique as required, and assists the patient in learning appropriate self-care techniques. The Licensed Practical Nurse completes all necessary documentation and participates in other activities as assigned.


LICENSED PRACTICAL NURSE ESSENTIAL DUTIES AND RESPONSIBILITIES

  • The LPN should make independent nursing visits on assigned patients.
  • The LPN should implement care plans and institute appropriate revisions as directed by the Registered Nurse on an ongoing basis.
  • The LPN should assists RN in performing specialized procedures as assigned.
  • The LPN should ensure physicians orders are carried out in a timely manner as assigned.
  • The LPN should reinforce teaching to patients and/or families as provided by the RN.
  • The LPN should prepare equipment and materials for treatment, observing aseptic technique as required.
  • The LPN should consult with RN regarding observations made which may necessitate referrals to other appropriate health care workers and/or agencies.
  • The LPN should comply with universal precautions and standard infection control protocols and general safety policies and procedures.
  • The LPN should maintain accurate and complete documentation of nursing activities on medical record.
  • The LPN should maintain close communications with RN case manager regarding general observations of patient and patient/family status.
  • The LPN should promptly inform the RN, physician or other persons participating in the patients care of changes in the condition and needs.
  • The LPN should participate in IDT meetings for Hospice as needed and contributes to care plan revisions of all patients reviewed Based on knowledge and experience.
. Requirements:

Minimum Qualifications

  • Graduate of an accredited school of licensed practical nursing.
  • Minimum of two years of clinical experience.
  • Current Wisconsin and Minnesota license required.
  • Current Wisconsin driver's license.

  • Ability to work well as part of a team and independently
  • Exhibit excellent written and verbal communication and organizational skills
