Job Summary
The RN Home Health provides care which is appropriate to the patient's diagnosis, symptoms, and physicians orders. This position directs and oversees care provided by Licensed Practical Nurse (LPN) and aides. The RN establishes and implements documented plans of care, determines appropriate outcomes/goals and communicates/collaborates with patient, interdisciplinary team, and physicians in providing care.
Supervised by the Supervisor and Manager of Home Health.
Essential Job Functions
Provides care which is appropriate to the patient's diagnoses, symptoms, and physician orders. Directs and oversees care provided by Licensed Practical Nurse (LPN) and aides.
Establishes and implements a documented plan of care. Determines appropriate outcomes/goals.
Communicates and collaborates with the patient, interdisciplinary team, and physicians in providing care.
Promotes patient/caregiver education using various verbal and written communication techniques that take into account the patient's/caregiver's cultural, ethnic, and personal needs or preferences
Documents patient/caregiver responses to care, treatment, and services, including progress towards goals.
Identifies discharge planning needs as early as possible and initiates needed referrals to other disciplines, alternative level of care, or community resources as needed.
Complies with Joint Commission, federal and state regulations, reimbursement guidelines, and Altru Health System and Altru Home Services policies and procedures.
Performs pain assessment and takes appropriate action according to standard operating procedure.
Performs other duties as assigned or needed to meet the needs of the department/organization.
