Educational Requirements

Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.

Position Requirements

A minimum of 6 months nursing experience within the past 3 years unless state regulations differ. Travel is necessary on a daily basis.

We are looking for experienced, talented, and compassionate Registered Nurse (RN) Case Managers to join our growing hospice team.



Patient care provided in the community, at home, or care facilities as appropriate.



Responsibilities include, but not limited to:



Coordinates and develops patient/family care and ensures the care plan is current, up to date, and reviewed at every team meeting.



Assesses and evaluates patients' level of care, needs, and requirements.



Manages a patient and family caseload which includes assessing, planning, implementaton, evaluating, and documentating the care provided.



Submits paperwork in a timely manner and updates and maintains the patient medical records.



Participates in regularly scheduled interdisiplinary team meetings to coordinate the care of patient and family, exchange information and problem solve, and receive staff support and education.



Ensure growth and profitability of the company through the responsible use of company resources, and educating the community of our services.



Evaluating outcomes of care.



Monitors that documentation from the LPN and home health aides are complete.



Ensures that employees are adequately oriented and trained to assignment, assists and encourages employees in developing their skills and self-confidence.



Applies specific criteria for admission and re-certification of hospice care to establish appropriate levels of care and the patient's eligibility.



Attends deaths of patients to assist and support family.



Consults and educates the patient/family regarding the disease process, self-care techniques, end-of-life and the processes for dealing with issues of ethical concern.



Uses case management approach and refers to other services as needed.



Evaluates own needs for support and uses identified systems to meet the need.



Meets or exceeds established productivity standards.



Participates in on-call coverage.



Accepts other assignments deemed as appropriate.
