This position comes with a $10,000 sign-on bonus!

· To provide clinical care for the hospice patient and family.

· To provide support to the RN Case Managers in the care of patients and support of families.

· On call 7 days on and 7 days off

· Provide admission nurse support, starting at 3pm when working 7 days on.

· Frequently communicate with the patient’s physician, the IDG, and the patient/family regarding the patient plan of care, pain/symptom management, and any concerns or issues as they arise, on an as needed basis.

Education and Experience Required
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors. Must complete and pass a state required background study.

· Able to communicate verbally and in writing to the extent required by the position. Able to physically perform the duties required by the position. Must have a car with required insurance coverage and a state driver’s license to travel to clients’ residences. Must have adequate computer skills.

· Current Registered Nurse licensure in the state of Minnesota

· Current BCLS certification

Key Responsibilities (Bring scope to this area and give details on each)

· Provide Care for Hospice Patients

Able to provide visits and clinical care/support to the hospice patient and family on an as needed basis to assist the RN Case Manager team. Accurate and comprehensive assessments completed at the time of the visit, with documentation completed in the home. Demonstrates knowledge of comfort medications, and hospice standing orders and their administration and indication. Utilize appropriate pain and symptom management techniques. Provide clinical education and ongoing support to the patient and family/caregiver. Supervise the hospice aide on an as needed basis. Participates in bereavement plans and hospice community events as able.

· IDG Communication

Communicate and collaborate with applicable members of the patients care team and the interdisciplinary team when providing care to the hospice patient/family. Refer patients’ needs appropriately to ancillary services available. Provide feedback and updates to the IDG following visits and care of the hospice patient/family.

· Customer First Attitude

Understands that the customer extends beyond the patient and family, including community partners and referral sources, etc. Consistently develops and sustains mutually beneficial working relationships. Is accessible and provides prompt resolution to concerns or problems. Encourages and facilities cooperation within the organization and with customer groups.

Adopts communication styles appropriate to the listeners and situation. Listens actively, asks questions, clarifies points and rephrases others’ statements to check mutual understanding. Practices accountability to daily work tasks, expectations and team norms. Identifies what needs to be done and proactively takes action to accomplish objectives and achieve company goals and priorities.
