Heartland Home Health Care a part of HCR ManorCare/ProMedica that provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy is currently searching for a Full time or part-time, 24, 32 hour or 40 hour per week Physical Therapist to support their home health patients. This opportunity allows you much independence and the opportunity to have successful outcomes in the home. No weekends or on call required.

As a therapist you are responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional Standards of Practice.

Educational Requirements

Bachelor's or Master's degree in physical therapy from an accredited institution. Currently licensed as a Physical Therapist in the state and in good standing with the Board.

Position Requirements

100% travel necessary on a daily basis. At least one year or more work experience in physical therapy in a multidisciplinary setting is preferred. Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients. Excellent communication skills, and ability to develop and maintain rapport with clients and families.
