We are looking for a Case Manager, focused on Rapid Re-Housing, to join our team and help us advance our mission.

As a Case Manager, you will work with program participants holistically and will help them access necessary social services, and act as a personal navigator for obtaining permanent housing. While working with our clients, this position will provide individualized support throughout the entire journey by helping each youth overcome any barriers, assess their supports, and set achievable long and short-term sustainable stabilization goals Come take this journey with us!

  • Job Type: Full-time
  • Work Schedule: Typically, Monday - Friday, must have flexibility to work a mixed schedule of days and evenings, and occasional weekends.

Knowledge, Skills and Abilities and or Competencies required to successfully perform the work:

  • Strong commitment to Diversity, Equity, and Inclusion.
  • Provide Case Management supportive services to youth in the Rapid Re-Housing Program.
  • Conduct individual assessments to determine needs and goals.
  • Assist youth in creating a housing plan.
  • Support youth in identifying and pursuing housing.
  • Support youth in moving into and maintaining housing.
  • Assist clients in meeting benchmarks in their goals.
  • Document youth outcomes in all appropriate databases.
  • Complete appropriate paperwork in a timely manner.
  • Develop and maintain relationships with landlords in the community.
  • Develop and maintain relationships agency-wide and community-wide with youth serving programs and agencies.
  • Prepare related reports including outcomes, successes, and challenges, including generating data for monthly reports.
  • Comfort with Microsoft Office applications as well as exhibit an ability to learn and utilize handheld applications systems and other electronic recording systems.
  • A high degree of professionalism and ability to work with and maintain confidential information.
  • Consistent ability to perform and contribute positively in a diverse, collaborative team environment.
  • Ability to coordinate care, delegate tasks, provide referrals and problem solve as needed.
  • Dynamic energy, ability to multi-task and negotiate multiple demands on attention.
  • Demonstrate an ability to utilize continuous improvement techniques and quality research methods to optimize overall program operation and build community relationships.
  • Ability to work efficiently and effectively both individually and as part of a professional team.
  • Must have a driving record that is acceptable to the company's Vehicle Use policy.

Required Education, Experience, Licensure, Certification required for entry into position:

  • Minimum of 1-2 years experience working with homeless or at-risk youth.
  • Valid Drivers License and current automobile insurance.
