DURATION: 12 Month/residency

PREREQUISTIES: Pharm.D. and hold an active pharmacy license or be eligible for licensure in North Dakota and Minnesota .

PURPOSE STATEMENT:

A pharmacist completing this residency will prepare to assume a staff position in an acute care hospital or ambulatory care setting, provide MTM services and long term care facility consulting, and provide consultative services to a variety of other health care entities. This pharmacist will develop teaching skills and be qualified to serve as adjunct faculty for a college of pharmacy, serve as a pharmacy student preceptor, or pursue a second year post-graduate training in a focused area of practice.

TRAINING PROGRAM- Goals/Objectives:

The objective of this residency is to provide graduates with the knowledge, training, and experience necessary to be able to identify and resolve medication-related problems. These experiences will occur in hospitals, long-term-care facilities, a regional hospice program, assisted living facilities, home health care and senior centers. The resident will become an effective pharmacotherapy decision maker and consultant. The resident will identify existing and potential drug-related problems and develop/initiate solutions. The resident will demonstrate the ability to monitor drug therapy of patients undergoing polypharmacy and communicate information/ recommendations to the healthcare professionals in a concise and meaningful fashion. The resident will develop and/or apply drug therapy audit criteria to evaluate the drug therapy of patients in hospitals and other healthcare facilities. The resident will apply their skills in a variety of practice settings and patient-specific situations, with the ultimate goal of improving patient outcomes. The resident will assist in the learning of pharmacy students enrolled in a pharmaceutical care lecture and laboratory courses at the NDSU School of Pharmacy. This coursework is designed to teach and assess practical application of the skills necessary to become a generalist pharmacist. In addition, the resident will have opportunities to lecture for students in the health professions and to assist with precepting students on advanced pharmacy practice experiences.

DESCRIPTION OF ACTIVITIES:

The resident will be involved in consultation services and Medication Therapy Management (MTM) at multiple sites and via Telepharmacy at LRH clinic sites. The resident will be involved in educational activities for health professionals. The resident will have the the opportunity for training in presentation skills through the facilitation of didactic lectures to NDSU School of Pharmacy students. The resident will assist in the learning of NDSU School of Pharmacy students in the Concept Pharmacy. The resident will assist with preceptor duties for students on advanced pharmacy practice experiences. The resident will attain a Teaching Certificate through the NDSU School of Pharmacy. The resident will be involved in all hospital pharmacy functions including distribution, order entry, monitoring, quality assurance programs, drug use evaluation studies, pharmacy & therapeutics committee functions, and individual case consultations. There will be participation in medical and nursing student training programs. The resident will be involved in public or community education programs. Special projects and research will be developed and completed by the resident and preceptor based on interest and need.
