HealthCare Support is currently seeking an experienced RN Case Manager to fill an opening with a Fortune 500 company Madison, WI.

The ideal candidate will have the following requirements for RN Case Manager:

• Two years' registered nurse experience
• Case management experience highly preferred
• EPIC experience preferred

Assignment details RN Case Manager:

• Review all patients identified for review to determine the medical necessity and appropriateness of enrollment in the care management program.
• Assesses all assigned patients and integrates family information from social work to collaboratively determine potential, actual and the next level of care needed
• Schedule/Hours: Monday-Friday Daytime Hours
