Starting Salary up to $47,000/year plus a $3,000 SIGN ON BONUS!

We are a dynamic team that provides Case Management services in the greater Milwaukee area for MyChoice Family Care Wisconsin. JFS provides supportive services that will strengthen families, children and individuals throughout their life cycle and within the context of their unique needs and traditions. We are looking for a certified social worker in the State of Wisconsin (or have a 4-year Bachelor's degree in social work, rehabilitation, psychology, etc.) with at least one-year of paid, formal (non-personal caregiving) experience working aging/elderly persons, persons with a physical or intellectual/developmental disability, etc. If you have the ability to work independently, meet deadlines (documentation, reporting, etc.), conduct assessments and provide supportive services to our clients with a professional and caring methodology, we would like to meet you.

Our case managers meet with individuals in their homes, complete strength-based assessments and develop customized care plans focused on client preferences. Our goal is to improve or maintain the clients' quality of life, independence, and overall health and safety. Working in tandem with a nurse cars manager. our staff collaboratively maintains relationships, coordinates all functional and social supports that promote independence and maintain client safety, and creates a plan of care to improve overall well-being is the core of our program.

This is a full-time, predominantly first shift position requiring a valid Wisconsin Driver's License, access to a vehicle, personal motor vehicle insurance (meeting minimum coverage limit requirements of the Agency), passing a Department of Justice Background Check and Federal Sex Offender Registry Check, and verification of references.
