We are looking for a Case Manager - Scattered Site Housing to join our team and help us advance our mission.

The Case Manager - Scattered Site Housing is a positive role model, mentor, and life coach for young adults of YouthLink who are entering into the housing program through Hearth Connections Coordinated Entry Referrals. The Case Manager helps residents identify and achieve goals that help them maintain safe and stable housing within the community. The Case Manager works to assist young adults to seek employment, further their education, and even explore training certificate trade programs to leverage their earning capacity. They accomplish this by building trusting relationships and focusing on residents' unique strengths and positive characteristics. The Case Manager guides residents toward personal empowerment, community connectedness, independence, and self-efficacy. This position contributes to resident success by keeping updated files through effective documentation and engaging with young adults using a Positive Youth Development Approach, and empowering them to reach their goals, while maintaining long-term sustainable success and stability.

Job Type: Full-Time

  • Work Schedule: 40 hours typically, Monday - Friday, must have flexibility to work in the day, evening and occasional weekends in case of emergencies.
  • $19-$22 per hour range

Knowledge, Skills and Abilities and or Competencies required to successfully perform the work:

  • Able to provide non-judgmental services, including demonstrating an acceptance of a variety of lifestyles, behaviors, and cultural and spiritual practices.
  • Strong knowledge in principle-based approach, strengths-based, and other areas reflected in YouthLink's Guiding Principles. Including harm-reduction, trauma-informed care, de-escalation tactics and MH/CD supportive resources
  • Knowledge of educational and vocational pathways for youth.
  • Ability to make sound decisions independently in crises.
  • Effective written, verbal, and interpersonal communication skills.
  • Comfortable with and effectively able to work independently.
  • Passion, understanding, belief and willingness to consistently incorporate strategic goals, agency principles, vision, mission and philanthropic objectives into daily work.
  • Strong commitment to Diversity, Equity, and Inclusion.
  • Comfortable with Microsoft Office applications and ability to utilize handheld application and systems.
  • A high degree of professionalism and ability to work with and maintain confidential information.
  • Able to work in a collaborative environment.
  • Demonstrated experience working with communities of color.
  • Knowledge of local housing regulations and the needs of youth experiencing homelessness.
  • Experience with case management documentation.


Required Education, Experience, Licensure, Certification required for entry into position:

  • 2-4 years of experience working in supportive housing, and knowledgeable about housing stabilization support services and working experience with at-risk, unsheltered youth, who may have some county involvement through the criminal justice system, probation, or previously aged out of the foster care system.
