Case Manager Responsibilities: Provide person-centered planning, support plan monitoring, coordination, referral/linkage, and advocacy related to waiver services and resources not funded through the waiver. Provide service with person-served access services and supports as needed to promote their quality of life, social, health, educational, vocational goals. Empower person-served to make informed choices. Responding to the constantly needs of the individuals assigned to you.

Requirements

  • Accredited four-year degree in social work, psychology, sociology, or a closely related field.
  • Effective communication skills, both written and verbal.
  • Excellent organizational and time management skills.
  • Excellent computer and typing skills.
  • Problem-solving skills and ability to multi-task.
  • Ability to work both independently and within a team.
  • Quick learner.
  • Ability to handle a challenge.
  • Caring and compassionate
  • Driver's License and car insurance. Role requires driving for client visits.
  • Successfully clear DHS background check.

Preferred Qualification:

  • Bilingual.
  • Familiarity with waiver programs and services.
  • One year of experience in ARHMS, ILS, IHS, care coordination, or case management.
  • One year of experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.

Schedule:

  • Monday through Friday 8:30 am to 5 pm.
  • Required office work during onboarding.
  • Case Manager will transition to working remotely once onboarding is completed.

Job Type: Full-time

Pay: $22.00 - $24.00 per hour

Benefit Summary:

  • Sign-On Bonus of $1500. Paid in the following intervals, $250 at the point of hire, $250 after 3 months, $1000 at 6 months.

Location:

  • Minimal office work after fully trained but must meet with clients within the Twin Cities Metro Area.

COVID-19 considerations:

  • Face masks are provided and required when meeting with individuals if training is required in the office.
