Rotating shifts starting between 7:30am-12:00pm working 40 hours per 2 week pay period

Summary:

The Pharmacy Technician Transitions of Care Specialist is a dedicated member of the Pharmacy Department team who is responsible for providing support to pharmacist-driven patient care activities as patients transition through the continuum of care, with a focus on admission to hospital, level of care changes during hospital admission, and discharge from hospital. Routine responsibilities include:
  • Completion of detailed and accurate prior-to admission medication histories
  • Identification and communication of barriers to medication access (e.g., uninsured, under insured) to appropriate care team members
  • Routine attendance at multidisciplinary care coordination rounds, facilitation of discharge medication procurement (e.g., promote utilization of onsite pharmacies
  • Enter/update demographic and prescription insurance information in software systems, facilitate patient signatures on forms needed to fill discharge prescriptions
  • Provide assistance with discharge medication payment collection and patient contact issues on day of discharge).
The employee will work closely with decentralized inpatient pharmacists, outpatient pharmacy technicians, outpatient pharmacists, and other members of the healthcare team to facilitate patient transitions of care. Other duties as assigned.

Responsibilities:

MEDICATION HISTORIES:
  • Prepares for, conducts, and documents accurate list of prior-to-admission medications for patients admitted to the hospital. This will include comprehensive review of prescription fill records, patient/caregiver/family member interviews, communication with offsite facilities, clinics, outpatient pharmacies, etc., utilization of third-party databases (e.g., Wisconsin PDMP, Wisconsin Immunization Registry, Forward Health), and comprehensive allergy review.
  • Demonstrate ability to communicate (written and verbal) barriers to medication history information, identifies, attempts to resolve discrepancies between medication information sources, and reports these to the pharmacist.
DISCHARGE PHARMACY UTILIZATION:
  • Promotes utilization of onsite discharge pharmacy at time of medication history collection.
  • Discusses benefits of onsite pharmacy utilization with patients who initially decline to use onsite services.
  • Fosters an "opt-out" culture with particular focus on high-risk patients.
MEDICATION PROCUREMENT LIAISON:
  • Serves as liaison between onsite outpatient pharmacy and the patient for any non-clinical issues related to discharge medications.
  • Responsible for obtaining and updating patient's insurance information in outpatient pharmacy software, follows-up with patients who have no form of payment for discharge medications at time of admission medication history, provides assistance with patient contact and payment collection issues on day of discharge.
  • For patients that report having no prescription insurance, investigates further and follows-up with social work/case management/financial counselors as needed.
  • Also serves as liaison between provider or inpatient pharmacist and the inpatient medication access specialists - inputting referrals for prior authorizations (PA) for discharge medications and staying up-to-date on status of PA or patient assistance program enrollment.
OUTPATIENT PHARMACY SERVICES:
  • Maintains up-to-date knowledge of Froedtert Health Outpatient Pharmacy services (e.g., Medication Access, Specialty Pharmacy) and promotes utilization of services to patients and other members of the healthcare team, particularly as it relates to prescription medication needs after the acute care stay.
  • Assists with facilitating enrollment in services when patients express interest.
CARE COORDINATION ROUNDS:
  • Actively participate in multidisciplinary care coordination rounds as the pharmacy team representative.
  • Provide information about medication-related barriers to discharge and facilitates solutions to these barriers as able.
METRICS/DATA:
  • Maintains active knowledge of discharge pharmacy utilization on nursing units most commonly assigned.
  • Communicates discharge pharmacy utilization rates to stakeholders (e.g., nursing unit leadership) with support of pharmacy leadership.
TRAINING:
  • Participate in the training of new team members, including pharmacy technicians, students, and pharmacists.

Experience:
  • A minimum of 3 years of experience in Pharmacy, Healthcare, Managed care, or other health insurance environment is required.
Education and Training:
  • High school diploma or equivalent is required. Associates degree preferred.
  • Graduate of ASHP accredited pharmacy technician training program preferred.
  • Certification as a Pharmacy Technician (CPhT) through passing the Pharmacy Technician Certification Board's exam (PTCB) or the National Health career Association's exam (ExCPT) is required within 6 months of hire.
  • Completion of the ASHP Medication Reconciliation Certificate is preferred.
