Description:

The Substance Use Disorder (SUD) Case Manager is responsible for providing case management services to adults with primary substance use concerns and/or behavioral health problems in Washington County. Wraparound services include finding resources to assist in stabilizing housing, securing employment, furthering education, connecting with follow-up treatment and other areas of need in the person's life.


SALARY: $22.60 - $23.55 an hour, depending on experience. Plus a $1,500 sign-on bonus!


ESSENTIAL FUNCTIONS:

  • Conduct an assessment to evaluate both the strengths and needs of individual.
  • Analyze situations, capabilities and problems to determine what services are required to meet the needs of the client.
  • Address the basic needs of the client, in addition to any therapeutic intervention, to increase effectiveness on issues such as housing, energy assistance and job security.
  • Formulate, implement, evaluate and revise goal oriented and strength-based treatment plan.
  • Develop a crisis/safety plan for the clients.
  • Maintain weekly contact with clients.
  • Develop a support team of formal and informal supports, which will meet monthly to monitor progress of goals and services.
  • Research new and unknown resources and utilize that knowledge of services/resources to assist the client and family in meeting their goals.
  • Safely transport clients as needed.
  • Collaborate with community partners and other community based substance use programs.
  • Ability to operate a motor vehicle safely and obey all traffic laws.
  • Makes recommendations for placement information to Substance Use Disorder Coordinator and Outpatient Services Supervisor. Fills out appropriate forms, and/or updates any changes in placement information to the Electronic Health Record.
  • Other duties as assigned.
Requirements:
  • Must have reliable transportation on a daily basis, a valid driver's license and automobile insurance.

Education and Training:

  • Must have a Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related field.
  • Position must keep current on information pertaining to client/family needs through seminars, trainings, media, text and research.

Experience:

  • At minimum, two years of previous experience working with individuals with substance use and/or mental health concerns.

Knowledge, Skills and Abilities:

  • Ability to read, speak and write clearly in English.
  • Ability to exercise good judgment and discretion in establishing work schedule.
  • Must be able to work cooperatively and collaboratively with other providers (Probation and Parole, treatment professionals, Children and Family Services workers, Washington County Human Services, Aging & Disability and Court Services).
  • Must establish and maintain productive and professional relationships with clients, colleagues, and other community agencies.
  • Must maintain client confidentiality.
  • Ability to prioritize and make appropriate decisions.
  • Ability to work independently and effectively manage time.
  • Must possess strong oral and written communication skills.
  • Ability to provide culturally, ethnically and gender sensitive services.
  • Ability to follow directions and implement recommendations.
  • Possess creativity in identifying problems and solutions.
  • Ability to self motivate and remain on task.
  • Knowledge and compliance with policies and procedures as detailed in the employee guide and manual.

Work Environment:

This position is housed within the Washington County Human Services Department with daily activities overseen by the Substance Use Disorder Coordinator and Outpatient Services Supervisor. This position operates in a professional office environment as well as within the community. This position is required to go into client homes, jails and any other setting necessary to provide services. This position routinely uses standard office equipment such as computers with various software programs, phones, photocopiers, shredders, filing cabinets and fax machines. Exposure to environmental conditions such as air conditioning, dust, odors, and outdoor weather conditions occur. Exposure to household pets may occur.


Physical Requirements:

While performing the duties of this position, the employee is regularly required to: converse and communicate with others; possess visual acuity which allows employee to view a computer monitor, read documents and safely operate a motor vehicle; remain in a stationary position as well as move about inside the office or other community settings (i.e. client homes, jails, county buildings); ascend and descend stairs; operate a computer and other office technology; and move objects weighing up to 20 pounds.


Travel:

Travel is primarily local during the business day.


Client Caseload and Hours of Work:

This position will maintain a caseload based on program need. The hours of work for this

position are varied, but typically are Monday through Friday during the business hours of 8:30 am and 4:30 pm. However, this position may be required to work evening hours on occasion outside of this timeframe based on the needs of the program.


Attendance:

This position requires regular and reliable punctuality and attendance.
