Palliative Care and Clinic Social Worker

Lakewood Health System is seeking a motivated Full-time Palliative Care and Clinic Social Worker!

This individual will be responsible to meet the needs of patient involved with our Palliative Care program. This person will work with other disciplines throughout Lakewood Health System, as well as resource planning to our patients/caregivers and families. Along with Palliative Care, this social worker will support clinic social work needs on behalf of our patients. Other roles will include collaboration of care with community resources, patient care teams, and community outreach needs.

This is a full-time position, working 64-80 hours every two weeks.
Qualifications

1. Bachelor's Degree in Social Work

2. State license as a professional Social Worker or eligible to be licensed

3. Minimum of 2 years relevant experience.
