Location: US:WI:MENOMONEE FALLS at our FROEDTERT MEN FALLS HOSPITAL facility.

1.0 FTE is Full-Time, any position less than 1.0 FTE is Part-Time, and .001 FTE is Optional Part-Time
Positions with an "FTE" (full time equivalency) of .8 - 1.0 are eligible for full-time medical, dental and vision benefits. Positions with an "FTE" of .5 - .799 are eligible for part-time pro-rated medical, dental and vision benefits
Shift: 1
Weekend on call, Saturday 8 AM -430 PM, and weekday first shift coverage as needed.
Summary:
Ability to assess and plan for interventions related to patient and family Psychosocial needs and appropriate community resources. Integrates assessment using information related to cultural, clinical, ethnic, age and other factors into the plan of care in order to implement discharge plans and interventions. Acts as active team member in the discharge planning process and primary responsibility for assessment of complicated social and financial factors and barriers to appropriate discharge and transitions to needed placements and resources. Assures that patient is referred to appropriate social and financial resources post discharge in order to transition patient efficiently and achieve the desired outcome. Refers patients/ families and care givers to appropriate resources regarding abuse/ neglect, domestic violence, alcohol and substance abuse and behavioral health needs. Performs appropriate clinical interventions related to grief counseling, bereavement, adjustment, crisis intervention in order to support the process of transition planning. Acts as a resource for staff and other members of the interdisciplinary team in relation to crisis intervention, bereavement, regulatory updates related to social work issues and complex case planning. Other duties as assigned.
Responsibilities:
One to five years of experience is required. One to three years experience in a medical / clinical setting is required. Five years of Medical social work, including discharge planning experience is preferred. Master's degree in Social Work from a CSWE accredited college or university is required. Current State of Wisconsin Social Worker Certification (CSW), or completion of State of Wisconsin Social Worker certification within 6 months of hire date is required.
Other information:
Clinical training in a medical/ clinical environment. Knowledge regarding management of complex discharges and processes. Knowledge of computers and good computer skills. Excellent communication and interpersonal skills, conflict management, organizational and planning skills.
