Case Manager II (Residential)
1. Maintain assigned Caseload. 2. Conduct individual & group counseling. 3. Complete written work on time, in accordance with regulatory & funding source guidelines, including, but not limited to: Social History, Treatment Plan, Progress Notes, Discharge Summary & required Reports. 4. Schedule required Meetings with Agent of Record (AOR)/Corrections representative. 5. Attend Staff meetings. 6. Monitor Resident's progress toward Treatment Plan Goals. 7. Hold Resident accountable to established guidelines. 8. Refer to collateral services (where indicated). 9. Attend training 10. Other duties as assigned. Other Duties and Responsibilities: 1. Fill-in for absent Staff. 2. Assist in training new Staff. Knowledge, Skills and Abilities: Cognitive therapy knowledge/skills that address the needs of the AODA/ Correctional Offender in individual and group counseling Minimal Qualifications: Bachelor's degree in Social Service field. Minimum of two years' counseling experience. Clear WI Caregiver Law. Pass Criminal Background Check. Pre-employment health and drug screening. Valid Driver's License, access to auto and auto insurance sufficient to meet agency requirements. pm21
