ProMedica Home Health Care, formerly Heartland Home Health Care is currently searching for a part-time nurse to support their patients in their homes. For this role we are looking for a RN to provide visits to our patients in the Green Bay and surrounding areas. This position requires providing visits during the week day hours. Weekend On Call rotation only averaging every 8th weekend.

As a RN you are responsible for the delivery of skilled nursing care and supportive services within the scope of the nurse's education and experience. Compliance with Heartland Corporate Standards and local, state, and federal laws, in an institutional setting, within the policies of the institution. Supplements nursing care of clients.

Educational Requirements

Currently licensed as a Registered Nurse in the state and in good standing with the Board in which he or she will practice.

Position Requirements

• A minimum of 6 months institutional nursing experience within the past 3 years, unless state regulations differ. The DON or Administrator may waive this experience requirement


• Registered Nurses providing skilled intervention for PASSPORT Patients must have 2 years experience as an RN.
