Position: Community Case Manager

Position Summary:


The Community Case Manager will provide services to individuals experiencing homelessness and who are in need of an immediate safe shelter option at either congregate warming rooms, or hotels. Permanent housing placement is the central focus of this position. Through case management support, individuals experiencing homelessness are aided in resolving their immediate basic needs and establish housing plans. Once individuals enter warming room options through Coordinated Entry and/or placement in a short term, subsidized hotel, they will be assigned to a community case manager. Each placement will be partnered with comprehensive community case management services and include crisis intervention, comprehensive assessments, and establishment of plans with goals focusing on housing, income and health and well-being. The Community Case Manager will maintain the client relationship and activities toward goal attainment until the consumer is safely housed, care is transferred to another provider or if the consumer chooses not to continue services. This position will utilize a person-centered, housing first approach and participate in our community's Coordinated Entry System

Essential Functions:

1. Using a trauma informed approach, provide comprehensive assessments, case planning and follow up services to individuals experiencing a housing crisis in a compassionate and dignified manner.

2. Provide direct individual, and group services with the objective of supporting consumers to return to safe housing-removing barriers and building assets to achieve their individual goals.

3. Assist consumers with accessing applicable community resources.

4. Build off the person's strengths to meet their individualized recovery needs. All services will be person centered and directed by the individual being served.

5. Utilize various methods to serve people; including, but not limited to: role modeling; monitoring; mentoring; supervision; assistance; and, cueing.

6. Ensure timely and accurate data entry and maintenance; complete annual training requirements for Homelessness Management Information System as well as any other agency required documentation.

7. Attend and participate in staff meetings, consumer reviews , in-service training, seminars, and conferences as required.

8. Work on site at hotel sites, shelters, community or where CCM consumers are residing.

9. Work collaboratively with system partners, like Coordinated Entry, Impact, Milwaukee County Housing Division, etc. representing the agency in a professional manner.

10. Responsible for maintaining flexibility in daily work schedule to adequately meet program needs.

11. Responsible for adhering to agency policies and procedures while carrying out daily responsibilities.

Other Duties and Responsibilities:

1. Ensure people are able to get transportation to all CCM related appointments

2. Other job-related duties as may be necessary to carry out the responsibilities of the position



Job Qualifications:

Minimal Qualifications: Bachelor's degree in social work, human services, or a like field is preferred. Two years of experience providing social services to people with mental health, substance use, and co-occurring needs and housing preferred.


Knowledge, Skills and Abilities: Significant knowledge of housing resources in Milwaukee County and direct experience serving people and advocating for housing needs . K nowledge and skills to work with a mental health and substance use recovery population; organization skills; experience in working with individuals in recovery who have mental health and/or substance use needs; ability to quickly develop rapport with people, excellent assessment and interviewing skills; good understanding of mental health needs; strong understanding of the substance use population; ability to work through difficult situations; flexible and able to multitask throughout the work day; effective use of time and resources to help maintain quality CCM services.


Physical Demands: Position is mobile with significant time spent in the community, including transporting consumers, meeting with consumers at shelter and or hotel locations, home visits and attending meetings; driving throughout Milwaukee County; must be able to go up and down stairs.
