Job Details

Schedule:
  • Every other weekend
  • 8-16 hours (opportunity to pick up additional hours)
  • Supporting our Fargo Hospice division

Scope:
  • Responsible for delivery of nursing services in accordance with the standards & practices of nursing, state & federal regulations, Accreditation Commission for Health Care (ACHC), and Ethos policies and procedures .
  • Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is maintained at all times .

  • Receive clinical direction and supervision from licensed Registered Nurses .
  • The foundation of hospice nursing is a complete skilled assessment, communicating with IDG and patient/family, and following the plan of care to meet the needs of the patient and the family during the terminal process .

Key Responsibilities:
  • Active Member of the IDG - Offer ongoing support and palliative care. Refer patient needs appropriately to ancillary services available. Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/ health care errors. Participate in case conferences as needed to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc. Support and maintain a culture of safety and quality.
  • Direct Client Care - Utilize physician orders and professional skills, provide care in accordance with hospice plan that meets each patient’s specific needs in compliance with federal and state regulations, ACHC, Ethos policies and procedures and reimbursement guidelines. Observing signs and symptoms and reporting to the Registered Nurse and/or Physician. Teaching and counseling the client and caregivers regarding the patients’ needs during end of life and other related problems of the client at home.
  • Client Records and Billing - Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required .

  • Communication - Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills .
  • Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

Education and Experience Required:
  • Must be licensed as a Licensed Practical Nurse in the state of North Dakota and Minnesota.

  • Must obtain CPR certification within 6 months from date of hire.
  • Must have a valid driver’s license and use of an insured personal vehicle .
  • Must complete and pass a state required background study .
  • Able to communicate verbally and in writing to the extent required by the position .
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, patients, families and facilities .
