SUMMARY The LPN Charge of Builsing provides support and direction to facility staff, ensuring 24-hour care is delivered per facility standards to assist each resident in achieving and maintaining their highest practical physical, mental and psychosocial well-being. PRIMARY RESPONSIBILITIES 1) Monitor nursing unit responsibilities, including leadership and oversight of nursing staff (RNs, LPNs, NARs, and TMAs). a. Communicate expectations and ensure that unit staff is carrying out their assignments. b. Monitor and mentor all unit staff, role modeling various expectations and regulations. Ensure documentation for reference to oncoming shifts details of residents' conditions, including but not limited to behaviors, fevers, falls and fractures. c. Assist with conflict resolution between residents, staff, families, community members, and other disciplines, providing education as requested. 2) Contribute to the comfort and well-being of the resident, ensuring respectful and dignified treatment, administering Nursing professional services as needed. a. Assist with managing challenging behaviors as care planned, to include any non-pharmacological interventions as needed. b. Provide a smooth transition for new residents through completion of the admission process and unit orientation as needed c. Initiate and conduct audits (i.e. call lights, charting, turning and repositioning, etc.), as needed 3) Make daily rounds to observe residents' physical and psychosocial status, assisting in physician calls to report and document changes in condition. Ensure families are kept updated as necessary. 4) Collaborate with the IDT (interdisciplinary team) to ensure adequate and appropriate flow of services. 5) Supervise dining room activities, assisting residents with eating as needed. 6) Maintain and complete all required documentation and charting on the computer or on forms in an accurate and timely manner according to policies and procedures. a. Accurately transcribe Physician's Orders as per facility protocol and forward necessary information to other departments. b. Initial Medication Administration Record (MAR) after medication is administered per facility policy and procedure, accurately documenting refusals for medication on back of MAR. 7) Support various other Nursing responsibilities as needed. JOB SPECIFICATIONS Education, Experience and Credentials Graduate of an accredited school of Nursing required. Minimum of two years of progressively responsible nursing experience preferred. Knowledge, Skills and Abilities Flexible and productive in an environment with regular interruptions. Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization. Possess effective problem solving skills. Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping. PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB Incumbent must be able to: Perform heavy work, lifting up to 50 pounds, and frequently lifting/carrying of objects weighing up to 20 pounds as needed. Push/pull residents in wheelchairs up to 200 pounds a distance of several feet as needed Effectively communicate, including following oral and written instructions, in order to communicate with people inside and outside the organization. Able to effectively use office equipment to review and create information. Work at a desk for extended periods of time and perform long hours of work sitting at a computer. Frequently stand and walk to move around office/building repeatedly throughout the day. Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed. Able to obtain reliable transportation TOOLS AND EQUIPMENT USED Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment. Use telephone, computer and standard office equipment. WORK ENVIRONMENT The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of residents, in responding to emergencies.
