Oncology Palliative Care RN Case Manager

Lakewood Health System has an opening for a full-time Oncology Palliative Care RN Case Manager at our Staples location!

The Oncology Palliative Care RN Case Manager will work directly with our Oncology patients by assessing, evaluating, and coordinating the plan of care for patients who are experiencing a debilitating chronic or life-threatening illness, condition, or injury. Duties attached to this position include but are not limited to:

1. Assessing the total patient.

2. Monitoring and managing serious and advanced illnesses and anticipates potential problems and intervenes based on assessment.

3. Coordinating the patient's plan of care with appropriate members and services.

4. Demonstrating the ability to exercise critical thinking skills in times of need and emergency situations.

5. Participating as a team member in the continuity and delivery of patient care.

This is a full-time position working 80 hours every 2 weeks.
Qualifications

1. Current MN RN licensure

2. Annual CPR training as required

3. Case Management experience is preferred

4. Oncology experience is preferred
