Scope:
  • Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Support and maintain a culture of safety and quality.
  • Work closely with prospective and current residents, families, responsible parties, and nursing home staff.
  • Responsible for the assessment of residents and their needs. Will assist in planning, coordinating and implementing changes to enhance the life, health and general well-being of the residents. 

Key Responsibilities:
  • Assist Patient in Transition Process - Meet with patient shortly after admission. Document and inform the IDT of patient concerns and needs. Collect social history and keep the history current and updated. Assist the IDT with formulating and implementing interventions for patients who have behavior problems or mood state issues. Assist the IDT with coordinating meeting related to conflict resolution and problem solving. Provide insight, alternatives and recommend interventions for resolution.

  • Complete & Maintain Documentation - Complete timely and thorough documentation and assessments. This includes: the admission IPN, psychosocial assessment, MDS, care plan, BIMS and PHQ-9 assessments, care conference IPN, and all other necessary documents. Update the patient psychosocial as needed and annually.
  • Address Resident’s Social & Emotional Concerns - Identify and seek ways to support resident’s individual needs and preferences, customary routines, concerns and choices. Participate in the resident care planning process. Formulate, review and update all care plans to reflect the resident’s current status and needs. Coordinate room changes. Assist the interdisciplinary team with formulating and implementing interventions for residents who have behavior problems or mood state issues.
  • Facilitate Conflict Resolution & Problem Solving - Coordinate meetings with residents, families, and/or staff when conflict is identified. Provide insight, alternatives and recommendations for resolution. Respect resident’s right to privacy and dignity. This includes the Grievance program.
  • Coordinate Discharge Procedure - Begin the discharge planning process at the time of admission by documenting in the resident chart information regarding lifestyle prior to admission, community resources utilized, and whether discharge is anticipated by resident or family. Work with the interdisciplinary team, resident, family, physician and community resources to develop a discharge plan that meets the needs of each individual. Assist resident in alternative arrangements such as transfers to other facilities, assisted living, etc. Coordinate discharge with community based services and provide necessary documentation for smooth transition. 
  • Provide Advocacy for Resident - Act as an advocate and educator for the patient, staff and family on the Bill of Rights. Provide information to the patient and/or responsible party regarding Advance Directives and explain the process for completing this document. Provide referral services to the patient and/or family related to legal or financial concerns. Assist with obtaining information related to community based services. Respect patient’s right to privacy and dignity.
  • Culture Change - Must participate in creating and maintaining a team approach to a culture in which the resident comes first. Is responsible for creating a home like environment for each person we serve. When coming into contact with a resident that has a need, address it and follow through until the need is met. Will receive on the job training from other departments to gain the tools necessary to serve our customers. 
  • Bereavement - Ability to perform a bereavement risk assessment for patient’s family and caregivers and carry out risk assessment findings, as applicable, at the start of care and following the death of the patient. Provides bereavement support and follow-up to family and caregivers. Represents agency at visitations and funerals, when applicable.

Education and Experience Required:
  • Degree in sociology, social work, nursing or a related field.
  • Must have strong assessment, problem solving, and organizational skills.

  • Must have familiarity with health and social resources, understanding of the psychosocial implications of acute/chronic illness, normal/abnormal aging process, relocation trauma for patients and families, and knowledge of basic medical terms.
  • Must have excellent oral and written communication skills, the ability to work effectively with others, and collaborate with the interdisciplinary team.
  • Must display genuine care for and interest in the elderly population and/or individuals with acute disabilities.
  • Demonstrate an ability to be flexible, organized and function under stressful situations.
  • General computer skills.
  • Must complete and pass a state required background study.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
