The Intensive Apartment Program (IAP) is designed to support individuals where they live in the community. The majority of individuals supported in the IAP live in the Metro Milwaukee area and are contracted through a Managed Care Organization (MCO) to provide Supported Independent Living (SIL) services. The following are services provided in the IAP:

Daily Living Skills Training - Teaching individuals skill development so that they may achieve greater independence. Tasks may include personal cares, dressing, food preparation, money management, safety practices, housekeeping, utilization of community resources and training on safety issues.

Daily Living Skills Maintenance - This service focuses on staff monitoring of the tasks that the individual has learned, as well as monitoring of skill retention, medications, medical appointments, and financial management. Some individuals may require some level of maintenance to maintain their level of independent living.

Supportive Home Care - These services are hands on tasks that may include personal cares, transferring, grocery shopping, toileting, medication administration, and nursing delegated tasks.

Transportation is provided by IAP staff to social activities, shopping and medical appointments. Staff need to be flexible when providing services and plan services and times of support with the individual and program director.

Documentation of the delivery of services as well as the progress of the individuals is recorded by the direct care staff in daily progress notes as well as on the electronic goal programs and EVV daily check-ins.
