Supervises the implementation of the case management program and ensures all care plans are carried out according to related policies and regulations in a healthcare facility. Overseeing the day-to-day activities and providing administrative and operational support to case managers; coordinating daily work of case management team. Reviewing and ensuring compliance with established case management policies, processes and all other regulatory requirements in a healthcare facility. Ensuring patients' medical, physical and mental needs are planned for and taken care of efficiently and effectively. Monitoring case management processes and identifying opportunities for improvement to senior managers.


Requires current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Case Management certification required within three years from date of hire. Must have a Bachelor's degree in Nursing. An Associate's degree in nursing plus Case Management Certification would be acceptable qualifications if hired prior to 2013. Must have a minimum of three years of clinical practice, with at least two years of case manager experience. Must demonstrate positive and proactive communication skills, as well as sound clinical and critical thinking skills. Incumbent must obtain and subsequently maintain required department specific competencies and certifications (BLS, ACLS) and when expected to achieve.
