Provides specialized counseling and support on healthcare and home care programs and services. Interviewing patients and their families and coordinating and planning programs and activities to meet their social and emotional needs. Providing crisis intervention and assisting families in understanding the implications and complexities of the medical situation and its impact on lifestyle. Coordinating healthcare programs among families, communities, and medical crews. Taking measures to improve awareness of community health and home care services.


This position is full-time, 80 hours every 2 weeks.


Licensed Social Worker (LSW) in the state of practice. Bachelor's degree in social work from an accredited curriculum with a strong preference for hospital or healthcare experience is required. The incumbent must apply the knowledge and skills of generalist social work practice, adhere to the National Association of Social Work Code of Ethics, and demonstrate knowledge of human behavior and developmental stages. Skills in verbal and written communication as well as establishing a rapport with patients, families, and the healthcare team are required. Critical thinking skills are necessary for independent judgment as well as for active participation with the healthcare team.
