Responsible for utilizing the professional nursing process to provide individualized nursing care. May perform rounds with or without physician in hospital and on hospital patients. May place verbal or telephone orders using read back. Abides by respective healthcare agency's policies. Collects and relays medical information (assessment, screening, diagnostic testing results and risk stratification) to providers on both hospital and clinic patients enrolled in the approved programs. Patient education is individualized to patient needs and condition, provider instruction. Care planning is individualized to patient needs and conditions and ensures it is updated. Assists with the coordination of patient care across the continuum by collaborating with physicians, advanced practice providers, staff and other professionals. Is a liaison with the healthcare team, and serves as a link to community resources. Works with the clinical team and may manage the disease registries by identifying and following up with patients not meeting clinical and evidenced-based goals as well as Sanford approved Clinical Practice Guidelines.

Requires current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Bachelor's degree in Nursing is required. Must have at least three years of clinical nursing experience. Must have extensive knowledge of nursing principles, practices and techniques. Must have knowledge of occupational health issues, Worker's Compensation regulations, workplace health and safety management, and case management.
