Performs basic patient care services within Enterprise Clinical Service initiatives.


Responsibilities may include but are not limited to the following: obtaining vital signs, gathering and providing contributing data to licensed health care professionals, registry management, point of care laboratory testing, EKG testing, and assisting with ambulatory procedures. May preform clerical tasks, schedule and coordinate patient appointments, coordinate providers patient panels by monitoring empanelment, performing pre-visit planning, and proactive health management that supports quality initiatives and improved patient outcomes. Potentially monitors and tracks patient referrals for health care services. May assist with supply, linen, and medication inventory management and will assist in upholding infection prevention principles throughout the ambulatory setting. Will help to meet the overall needs of ambulatory patients. The Care Team Associate has knowledge of and utilizes appropriate age-related patient care standards relating to the physical and psychological needs of patients from pediatrics through geriatrics. Must be self-motivated, be both independent and able to work well within a team, and is passionate about providing exceptional care to patients. Knowledge of medical terminology and medical equipment/supplies preferred but not required.


Whether working in Minnesota or North Dakota, certification is highly preferred. If working in North Dakota and not a Certified Nursing Assistant, must be registered as a Nurse Assistant through the North Dakota Department of Health within four months from the date of initial employment. Certification is required if designated by law (i.e. Home Care). HS Diploma or equivalent
