Job Description: Under the direction of the East Region Leadership and in collaboration with the Essentia Health Community Health Program Manager, the Community Health Specialist will facilitate strategies addressing community health needs. This position will coordinate a Community Health Committee within the local hospital and make progress on the hospital's Community Health Needs Assessment Implementation Plan. This position will work side by side with local community health stakeholders and partners, school districts and community members to affect change that will support residents' health in the areas identified by the community health needs assessment at each Essentia Health Hospital.

•Collaborates with community members and partners to increase awareness of and participation in community health interventions through seminars, presentations, key stakeholder meetings, marketing, etc.
•Coordinates with marketing/PR to host community health events as applicable to community health initiative(s), represents Essentia Health at community events and internal events
•Coordinates an internal hospital Community Health Needs Assessment Committee to review community health strategies and approve funding allocation.
•Works closely with the Community Health Program Manager to evaluate impact of community health interventions.
•Reports progress on community health initiatives to internal Essentia Health leaders and to community partners.
•Works with the Community Health Program Manager to complete the Community Health Needs Assessment for the Essentia Health local hospital.
•Assures all standards of care and recognition criteria (if applicable) are achieved by community health initiative(s)
•Upholds Essentia Health's mission, vision, values and ethical standards and demonstrates the behavioral and service expectations as defined in our policies and procedures

Education Qualifications:
Bachelor's degree in Health Education, Health Policy, Nursing, or related field with strong critical thinking, complex problem solving, community building and leadership skills.
