Job Title: Senior Behavioral Health Case Management | Health Plan | Multi-City

JOB SUMMARY

The position will sit in Fargo or Bismarck

Provides assistance to patient populations with social, economic, developmental, emotional and related issues. Also provides the psychiatrist with inpatient consultative services. Referrals to the service are made by physician or other healthcare professionals. Develops programs and services to meet the needs of various populations. Develops the psycho-social plan of care, and provides clinical screening, assessment and referral services to meet the needs of various patient populations (i.e., oncology, detox, post-partum, bariatric, other) with mental health/counseling needs; provides case management functions for appropriate populations; designs plan of care and provides follow-up; and coordinates care conferences. Works with all age groups and families, including children, adolescents, adults and geriatrics.

QUALIFICATIONS

Must have current licensure to provide counseling/therapy services in the state of practice. MSW, MA, MS or greater required.
