JOB SUMMARY
Provide safe, quality home care and hospice services by performing personal care and designated health-related services to patient under the direction and supervision of the registered nurse or therapist.
POSITION RESPONSIBILITIES
Home Health Hospice HHA/CNA
home health and hospice patients seeing them in there own home, assisted living and nursing home.
DEPARTMENT DETAILS
QUALIFICATIONS
Minnesota Home Health Aide Certification registered with the Minnesota Department of Health in addition to CNA Certification as a Nursing Assistant is preferred. Must be able to meet Federal Registry requirement as a Home Health Aide. The incumbent must have a valid driver's license and access to an insured vehicle. High school graduate or equivalency is preferred. Must have attended a State-established 75 hour home health aide training, which includes at least 16 hours devoted to supervised practical training and have passed the competency evaluation. Individual must have worked for compensation in the last 24 consecutive months as a nurse aide. Six months experience as a nurse aide in an acute care or long-term facility or as home health aide in a home health agency is preferred. Knowledge of health care environment is preferred.
