Responsible for carrying out all the duties related to hospital pharmacy operations and assisting the director of pharmacy in policy and procedural matters.


JOB RELATIONSHIPS:

A. REPORTS TO: Director of Pharmacy

B. SUPERVISES: Daily authority over supporting activities of pharmacy technicians.

C. INTERRELATIONSHIPS WITH: Pharmacy staff, physicians, nurses, and other hospital staff as appropriate.


HOURS OF WORK: 40 hours/week or as necessary to complete work, rotating weekends, and holidays.


DRESS CODE: Professional attire


ESSENTIAL REQUIREMENTS OF WORK (Minimum qualifications necessary to function at full productivity).

A. EDUCATION AND TRAINING: Bachelor of Science degree in Pharmacy or Pharm.D degree. License to practice pharmacy in Minnesota.

B. EXPERIENCE: Prefer hospital pharmacy experience.

C. JOB KNOWLEDGE (Specific): Inherent in degree


BUDGET: Submit budget requests to director of pharmacy and participate in budgeting process as requested.


CLIENTELE DIRECTLY AFFECTED BY JOB: Nursing staff, patients, medical staff, and other pharmacy personnel.


PHYSICAL DEMANDS OF THE JOB: Many hours on feet, moderate lifting of less than 25 pounds, many hours spent viewing computer monitors.


PHYSICAL ENVIRONMENT OF THE JOB: Clean, well lighted, good ventilation and temperature.


EQUIPMENT OR MACHINES USED ON THE JOB: Electric typewriter, computer, printers, Omnicell dispensing machines, balance, IV compounding hood.


MENTAL DEMANDS OF THE JOB:

A. JUDGEMENT AND INITIATIVE: Very important--professional decisions regarding patient care must be made often without consultation. Initiative in work will determine overall effectiveness. Must be able to identify and manage clinical needs of hospitalized patients. Employee must be able to work cooperatively with other clinical professionals in the development, production, and communication of necessary clinical information.

B. INDEPENDENT ACTION: Must be familiar with all aspects of the department so the person can represent the department as necessary in day to day decisions in answering questions.

C. EFFECT OF ERROR: May cause patient harm.

D. CONTACT WITH OTHERS: Nursing, medical staff, other departments in hospital. Models professional behaviors and demonstrates commitment to the mission, vision, and philosophy of Rice.


PRINCIPAL JOB RESPONSIBILITIES, TASKS, AND AUTHORITIES:

A. RESPONSIBILITY: Provide the right drug to the right patient at the right time.

PRIORITY: 1

TASKS: Immediately process medication orders as they arrive in pharmacy

B. RESPONSIBILITY: Conduct supportive professional activities as directed.

PRIORITY: 2

TASKS: May include: Multi-disciplinary rounds Medication monitoring, such as labs, kinetics, warfarin dosing, renal dosing Parkinson's clinic MS Clinic Falls Clinic CVHR Dialysis care conferences Hospice

Parenteral nutrition consulting Long-Term care consultation Educational presentations for physicians, hospital staff, and community Committee work promoting patient care and safety Serve as preceptor for pharmacy students

C. RESPONSIBILITY: Assist other staff as necessary to complete daily work.

PRIORITY: 2

D. RESPONSIBILITY: Continuous Quality Improvement

TASKS: 1. Demonstrates understanding of the CQI philosophy 2. Participates in CQI training activities or CQI teams if appropriate 3. Demonstrates sensitivity to customers and their needs 4. Interacts appropriately with internal customers, i.e. coworkers

within department, staff across departments. 5. Interacts appropriately with external customers, i.e. patients,

families, medical staff, vendors. 6. Role models positive behaviors.

E. RESPONSIBILITY: Service Excellence

TASKS: Demonstrates an ongoing commitment to the Service Excellence philosophy by adhering to behaviors outlined in Rice's "Standards of Excellence."

F. RESPONSIBILITY: Safety - Maintain and promote a safe environment for all patients, visitors and staff.

TASKS: A. Consistently follow all policies, practices and work rules. B. Do not use shortcuts or "work-arounds" that may reduce safety or increase

risk. C. Stay alert, act responsibly and use common sense to reduce risks. D. Report "actual events" and "good catches" as soon as possible. E. Create a safe environment by eliminating hazards and identifying and

reporting unsafe systems. F. Complete all mandatory safety education, attend safety sessions, review

and understand the Safety Program (available on RiceNet). Seek answers to questions you have about the Safety Program.
