Job Details

The Licensed Practical Nurse assists the Registered Nurse in the delivery of nursing care, coordinates and monitors Nursing Assistant practice and makes assignments within established guidelines. Required to pass medications and perform treatments within nursing scope of practice. Must have every other year up dates and able to perform in emergency.


JOB RELATIONSHIPS:

A. REPORTS TO: The LPN reports directly to the Clinical Coordinator with household accountability to the Household Coordinator.

B. MONITORS WORK PERFORMANCE OF: Care Assistants

C. INTERRELATIONSHIPS WITH: All nursing staff, physicians and various departments within the facility.


HOURS OF WORK: Hours can change based upon the needs of Household residents.


DRESS CODE: Name badge, Uniform top & slacks.


ESSENTIAL REQUIREMENTS OF WORK (Minimum qualifications necessary to function at full productivity).

A. EDUCATION AND TRAINING: Graduate from an accredited school of practical nursing. Must maintain current license as an LPN in good standing with the Minnesota State Board of Nursing.

B. EXPERIENCE: Experience working with elderly in a long-term care setting.

C. JOB KNOWLEDGE (Specific): Working knowledge of practical nursing skills, ability to communicate orally and in writing, ability to establish effective working relationships with residents, co-workers and families. A specific understanding of the cognitive, physical and emotional changes experienced in the geriatric population. An understanding of the range of treatment and medications needed by residents of all ages in relation to the developmental aging process, with a high focus of attention towards the geriatric population.

PHYSICAL DEMANDS OF THE JOB: Sustained physical effort, seldom seated. Continuous activity; pushing, lifting, pulling, etc.


PHYSICAL ENVIRONMENT OF THE JOB: Fair working conditions with frequent exposure to disagreeable elements and danger. Contact with body fluids and some chemical substances.


EQUIPMENT OR MACHINES USED ON THE JOB: Uses transfer belts, lifts, telephone, fax, medicine cart, blood pressure cuffs, thermometer, blood glucose monitor, suction machine and oxygen tanks, I.
• pumps, tube feeding pumps, and oximeter..


MENTAL DEMANDS OF THE JOB:

PRINCIPAL JOB RESPONSIBILITIES, TASKS, AND AUTHORITIES:

A. RESPONSIBILITY: Medication Administration and Assists in Providing Nursing Care to Residents through use of the Nursing Process. (Charting & Updating Care Plan)

PRIORITY: A % OF TIME: 70% DISCRETION: A

TASKS:

1. Passes oral medications, gives intramuscular and subcutaneous injection according to RCC policy.

Assist with administration of flu vaccines and mantoux tests. 2. Must have a thorough understanding of the medications he/she passes

and must be aware of potential side effects of medications; must report side effects.

3. Glucose testing and administer insulin as established by RCC policy.

4. As authorized by RCC policy and with proper training, the Licensed Practical Nurse may set up IV fluids to be started, hang IV fluids, monitor and discontinue the fluid. Also may change tubing, and perform central line dressing changes.

5. Assist with all wound care. 6. Administer oxygen as ordered by the physician. 7. Assist with and perform indwelling catheter insertion and removal. 8. Assists in assessment of physical, psychological and social dimensions of

residents. 9. Assists in planning a formal plan of care. 10. Provides input in implementing care.

11. Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures.

12. Intervenes with proper technique, procedure and safety precautions to meet individual needs of residents.

13. Assists in evaluating nursing care provided. 14. Needs to demonstrate efficiency with weekly charting and updating of care. 15. Receives a thorough report from NA/R assigned to care for a particular resident and receives report through use of the resident care plan. 16. Updates and makes recommendations to the care plan as needed. 17. Reports changes in resident condition to Charge Nurse/ Resident Nurse Manager by frequent updates and timely intervention. 18. Demonstrates efficiency with incidental and follow-up documentation. 19. The individual resident plan of care must be reviewed and approved by an RN. 20. Participates in staff assignments and their revision. 21. Replaces ill calls.

TASKS:

1. Maintain an environment that achieves high quality care (physical, mental, emotional, social and spiritual) of residents and maintain their independence and dignity to the highest degree possible.

2. Respect the Household as the resident's home. 3. Assist the resident in feeling a sense of belonging, ownership and control within their

home. 4. Contribute to making the Household a healthy and happy place. 5. Promote activities that help residents achieve their best day, filled with a wide variety

of meaningful relationships and activities. 6. Maintain effective communication systems with all customers.

LEVEL OF AUTHORITY: C

C. RESPONSIBILITY: Customer Relations (Admissions/Tours)

TASKS: 1. Welcome and greet new residents, families and friends. 2. Ensure that resident's room is ready for receiving the resident. 3. Assist resident's families with moving in, unpacking belongings, etc. 4. Participate in the orientation of the new resident and family members/friends to the

Household. 5. Help resident feel comfortable and welcomed. 6. Maintain a working knowledge of RCC's programs and services. 7. Provide answers to questions and provide assistance to visitors of the Household. 8. Provide tours of the Household to visitors, etc.

D. RESPONSIBILITY: Customer Meals and Dining

TASKS: 1. Assist in preparing, storing, recording and serving meals, beverages and snacks for

residents/customers based upon resident requests, regulations, established polices procedures and best practices.

2. Set and clear dining tables, assist residents with menu selection, meal and beverage service, and clean the dining room according to established procedures and schedules.

3. Clean dishes, equipment and work area according to established procedures and schedules.

4. Create a positive dining experience for customers that promote their independence, choice and overall well being.

5. Assist residents to eat as needed. 6. Encourage resident participation in the dining program, and meal activities.

E. RESPONSIBILITY: Household Environment

TASKS 1. Maintain uncompromising levels of cleanliness in the Household. 2. Maintain an attractive, comfortable, and clean home in an environment consistent

with regulations and established policies and procedures and best practices.

3. Maintain clean spaces in the Household, including resident rooms, Household common spaces, as well as support spaces and equipment.

4. Respect resident time and property; treat them with courtesy, and respect their personal belongings.

5. When working in resident areas, you are a guest in their home. Support all aspects of the Household environment, including pets, plants, children, visitors and guests, facilitating all planned and spontaneous needs and activities.

6. Coordinate work with the Household team to ensure the housekeeping needs of the Household are met within the staffing resources of the Household.

7. Maintain supply of all materials to meet resident needs in the Household.

F. RESPONSIBILITY: Life Enhancement (Activities and Social Services)

TASKS:

1. Identify, implement and incorporate resident daily pleasures and interests into Household life based upon input from residents, families and friends.

2. Lead and participate in Chat Circles with residents and staff in the Household daily. 3. Assist residents with activities of their choice, group, individual or one on one. 4. Provide for resident leisure life activities. Take residents outdoors, or out in the

facility community. 5. Participate in outings and special events. 6. Engage residents individually or as a group to eliminate boredom, helplessness

and loneliness. 7. Perform miscellaneous tasks as needed for residents such as shopping, running

errands, assisting with correspondence, reading/writing letters etc.

8. Provide one on one interaction with residents based on preferences. 9. Transport residents to other parts of the facility (ie. Beauty shop, dining room, etc).

G. RESPONSIBILITY: Team Participation

1. As a member of the team, actively participate in team activities, meetings and processes.

2. Promote team building concepts. 3. Facilitate involvement in Chat Circles, and other Household events with residents,

coworkers and visitors. 4. Assist in the interviewing, selection and orientation of new team members as

assigned. 5. Represent the Household issues at community functions. 6. Effectively collaborate with staff to promote the residents' total well being. 7. Act as an advocate for Household residents, families and friends internally and

externally. 8. Always provides support, friendship and kindness for each of the household

members.

H. RESPONSIBILITY: Other Direct Resident Care Activities

TASKS:

1. Gives direct care to residents with complicated nursing problems and assists with residents in unstable conditions.

2. Performs treatments or assists in treatments according to nursing policies and procedures.

3. Provides appropriate and attentive pain management which includes: a. Accepts the residents report of pain.

b. Relieves the physical and psychosocial symptoms associated with pain while optimizing individual and family functioning. c. Promotes active participation by residents and their families in selecting pain management strategies and in communicating new or unrelieved pain.

d. Provides consistent state of the art care through multi-disciplinary care team and universal guidelines for pain management.

e. Assists in educating residents and families about pain management.

4. Assists with answering call lights as needed. 5. Assists with meal. Monitors meal process, serve and feed as necessary.

6. Follows up on observations reported by NA/R's. 7. Listens to nursing report regarding residents and gives nursing report as needed. 8. Receives reports from NA/R's and reports findings to Charge Nurse. 9. Notifies physician regarding care needs of residents, clarification of orders via fax or telephone. 10. Orders medications from pharmacies, checks cupboard on med room for supply PRN.

1. Assists with resident and family education. 2. Provides support for emotional needs. 3. Is familiar with and utilizes community resources as needed to meet

resident needs.

J. RESPONSIBILITY: Monitoring of Resident Care.

TASKS:

1. Make rounds on residents. 2. Observe privacy and dignity issues as they relate to resident care.

3. Monitors work performance of NA/R's. If employee counseling report needed, initiates and documents accordingly. Follows up with employee on related incidents. 4. Assists in evaluation process of NA/R's.

K. RESPONSIBILITY: Other Activities Related to Resident Care

TASKS:

1. Develops and maintains interpersonal relationships with residents, visitors, and families. 2. Keeps resident information confidential and respect resident's right to

privacy. 3. Assists in maintaining a clean, safe environment for residents and co-

workers. 4. Is familiar with safety and emergency disaster plans.

5. Participates in inservice programs for self-enrichment to maintain a high standard of nursing practice, and seeks further growth with other educational opportunities. Attends nursing staff meetings.

TASKS:

TASKS: 1. Acts as an advocate for Resident Bill of Rights 2. Assures that all residents, families and staff are aware of resident rights 3. Assists in the identification of vulnerable adult issues 4. Respects resident property 5. Maintains dignity and privacy of residents by knocking before entering resident room, addressing them in a respectful manner, discussing their status in a private manner, treating them in a courteous and polite manner.
