JOB SUMMARY

The Health and Wellness Coach provides expertise in the development, implementation, and evaluation of all levels of health and wellness programs for individuals and groups; facilitates lifestyle change through a combination of assessments, interventions, behavior change strategies, health enhancement and maintenance strategies that create environments that support good health practices; works collaboratively with physicians and other healthcare providers by developing treatment plans that facilitate lifestyle changes through a combination of efforts; provides expertise and support in the evolution of health and wellness programs to align patient and community needs with appropriate resources; assists with the marketing and promoting of services, and represents Altru Health System in educating our internal and external audiences regarding the classes, services, and programs; and maintains excellent customer service skills to allow promotion of a superior experience. Patients cared for include children, adolescents, adults and geriatric.

Primary Job Duties:

- Reviews and interprets medical status (i.e., blood pressure, heart rate, respiration rate, color and mentation) and wellness data to provide recommendations, identify abnormal responses and take appropriate action.

- Performs fitness assessments to include but not limited to aerobic capacity, body composition, vital capacities, strength, flexibility, and girth measurements.

- Develops and instructs individuals on therapeutic exercise with direction from the therapist or provider regarding specific patient needs and realistic goals (i.e., Bariatric, Diabetes, Cardiac Phase III, etc.).

- Helps client develop realistic, engaging, and measurable goals to be accomplished in identifiable timelines moving the client toward their values-based vision.

- Conducts appropriate internal and external evaluation and development of programs that meet the customer's and community's needs as well as the organization's strategic objectives.

- Monitors program effectiveness and restructures programs by continually evaluating outcomes and member response to current and new programs and services.

- Applies innovative, effective and engaging delivery methods for programs/services for individuals and/or groups.

- Provides feedback and suggestions for change to the therapist/provider regarding individual's response to exercise or services and refers individuals to appropriate health care providers, when appropriate.

- Performs and documents calibration, maintenance, and cleaning of supplies and equipment.

- Represents Altru Health System in the effective marketing of programs and services offered.

- Communicates proactively with participants, services delivery partners, physicians, team members and other health professionals to provide case coordination and develop a plan of care that facilitates the efficient use of health care resources.

- Determines and provides relevant community and/or healthcare resources that support the individual with effective and efficient medical utilization.

- Promotes wellness and provides education regarding preventive care measures.

- Works collaboratively within a team setting to provide positive outcomes for individuals, peers, and overall operations.

KNOWLEDGE & SKILLS REQUIRED :



Required:

• Bachelor's Degree in exercise science, exercise physiology, athletic training, sports medicine or a related field.

• Certified as a Clinical Exercise Specialist by the American College of Sports Medicine (ACSM) or Certified as an Advanced Health and Fitness Specialist by the American Council on Exercise (ACE) or will become certified within 18 months from date of hire.

• Past experience in fitness, exercise prescription, and/or sports/athletics.

• Demonstrated ability to be outwardly engaging, approachable and flexible.

• Demonstrated ability to motivate people and identify, coordinate and apply resources effectively to achieve desired outcomes.

• Successful completion of the functional assessment.

Preferred:

• Achieve health coach certification within 18 months from date of hire
