JOB SUMMARY

Responsible for the coordination of care, patient assessment, patient education, triage, and various other interventions. Requires excellent time management skills and ability to prioritize/triage the needs of the patients and the practice. Occasionally works in emergent patient care situations requiring critical thinking and prompt assessment skills. Will perform age related patient assessment and intervention as well as scheduling tests and interventions per provider order. Responsible for utilizing the professional nursing process to provide individualized nursing care to patients across age continuum in an ambulatory setting. Cares for patients in all phases of specific states including assessment, disease maintenance, treatment, and follow-up as patients move in both inpatient and outpatient setting. POSITION RESPONSIBILITIES

Builds a report with the patient and explains the condition and course of action effectively. Delivers written and oral communication, responds to questions and concerns and produces specific outcomes and impact. Performing age related patient assessments and nursing interventions. Performs triage, sorting patients needing urgent care from those with less severe injuries. Provides direct service to internal and external customers. Responsible for promoting a safe environment for patients, families, staff residents and visitors. Working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients. Assists medical staff using less common devices and tools to treat serious illnesses and injuries. Coordination and management of care for patients, including with complex needs. Effectively delegates and supervises based on one’s state practice act and an understanding of the concepts of responsibility, authority, and accountability. Explains treatments and procedures to patients suffering less serious illnesses and injuries. Takes patient history or vital signs and uses this information to assist in patient prioritization. Utilizes specific resources for checking and cross-checking medical orders given.

DEPARTMENT DETAILS

RN will work both ambulatory and inpatient procedures.

QUALIFICATIONS

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific licensure, competencies and certifications. Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelors Degree in nursing preferred.
