The Renal Dialysis Patient Care Coordinator (PCC) is a Registered Nurse working under the supervision of the Manager and Regional Supervisor of Renal Dialysis. The PCC assures the units requirements and needs are being met in relation to patient care, Quality Assurance and Performance Improvement (QAPI), scheduling and other unit operations.

ESSENTIAL JOB FUNCTIONS
  1. Supports and models behaviors consistent with the mission, vision and values of Altru Health System.
  2. Performs pain assessment and takes appropriate action according to standard operating procedure.
  3. Collaborates with the multidisciplinary team, including the social worker, dietician, nephrologist, and nursing, as well as referring facilities and family members to obtain a holistic view of the patient through assessment of the physical, mental, and emotional status of the patient.
  4. Identifies, conducts follow-up and takes action on unsatisfactory outcomes, unusual findings, special needs and requests.
  5. Monitors patient's outcomes, conducts variance analysis and manages achievement of consistent and/or expected clinical outcomes.
  6. Provides patient care, maintaining clinical skills, providing instruction and functioning as a resource within the department.
  7. Responds to and coordinates emergency situations within the department.
  8. Assures completion of required documentation, data/sample collection/entry, and disinfect procedures according to standard operating procedure.
  9. Assists in providing professional growth and educational opportunities to staff and coordinates student experiences.
  10. Coordinates staff and patient schedules to meet the needs of the department.
  11. Performs other duties as assigned or needed to meet the needs of the department/organization.

KNOWLEDGE & SKILLS
Required:
  • Registered Nurse licensed by the North Dakota Board of Nursing or licensed through the "Compact State Agreement." *Only for Grand Forks and Devils Lake locations.*
  • Registered Nurse licensed by the Minnesota Board of Nursing. *Only for Roseau and Crookston locations.*
  • Must obtain Minnesota license from the Minnesota Board of Nursing within eight (8) weeks from the first day of work. (Cannot practice care in Minnesota until licensed which includes phone calls.) *Only for Grand Forks location.*
  • Must obtain North Dakota license North Dakota Board of Nursing within eight (8) weeks from the first day of work. (Cannot practice care in North Dakota until licensed which includes phone calls.) *Only for Crookston location.*
  • A minimum of three (3) years of clinical experience as a Registered Nurse and one (1) year of dialysis experience.
  • Demonstrates the ability to effectively communicate both verbally and in written format.
  • Successful completion of the functional assessment.
Preferred:
  • Bachelor's Degree in Nursing.
