This position will support the LeGrave on First housing project in downtown Grand Forks: http://www.thegfha.org/housing-first.html

The Nurse Case Manager reviews system admissions: assesses the patients to identify needs; ensures that patient care plans are being followed and appropriate services are administered; educates patients and families about the physical processes of their illness and medications; educates patients on technical procedures pertinent to their illness within the scope of their nursing license; establishes discharge planning consistent with overall outcomes/goals; collaborates with community resource persons, primary provider and multidisciplinary team; collaborates in establishing and maintaining case management program goals; assists in developing clinical guidelines; maintains knowledge in reimbursement; assists the system with insurance verification and reimbursement issues; assesses all short stays; communicates regarding appropriateness of admission and/or need for continued stay; collects and analyzes data; reports analyzed data; acts as a liaison/resource regarding quality improvement program data and completes all required documentation. Patients cared for include infants, children, adolescents, adult and geriatric.

KNOWLEDGE & SKILLS REQUIRED :

Required:
• Registered Nurse.
• Licensed by the North Dakota Board of Nursing or Compact State
• Nurses who provide nursing care to patients calling from Minnesota will need to be licensed by the Minnesota Board of Nursing or will obtain their Minnesota license within 2 months from their first day of work (telephone calls from patients calling from Minnesota can not be taken from any nurse unless they have obtained their Minnesota license).
• Demonstrates effective verbal and written communication skills and the ability to be outwardly engaging, approachable and flexible.
• Demonstrable ability to motivate people and identify, coordinate and apply resources effectively.
• A minimum of three (3) years of experience in a health care setting.
• Must demonstrate ability to multi-task and organize priorities.
