JOB SUMMARY

Integrates health care for clients from preadmission to discharge. Coordination and sequencing of the clients care. Integration enhances patient flow and interdisciplinary communication promotes early intervention. The nurse seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data. The nurse functions in structured and unstructured health care settings described as a geographic and/or situational environment that may not have established policies and procedures. The nurse utilizes independent nursing judgment when integrating health care. The nurse has knowledge of and utilizes appropriate age-related patient care protocols relating to the physical and psychological needs of pediatric, adolescent, adult and geriatric patients according to department scope of service. The nurse serves as a key link between physicians, clinics, insurance concerns, communication, and inpatient floors. Establishes and maintains good interpersonal relationships with those with whom has contact. Able to work with growth and development needs of client populations in clinical area. Strong communication skills are required. Coordinates, facilitates and negotiates with others. Monitors, evaluates and trends patient responses utilizing structured care methodologies. Must work closely with ancillary and support services. Ability to solve problems. Self-motivated, working with little direction. Ability to facilitate groups and implement projects when necessary. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

POSITION RESPONSIBILITIES

Assists in assessing risks, benefits and consideration of alternatives. Coordination and management of care for patients, including with complex needs. Delivers written and oral communication, responds to their questions and concerns, and produce specific outcomes and impact. Effectively delegates and supervises based on ones state practice act and an understanding of the concepts of responsibility, authority, and accountability. Examines a specific problem and understands the perspective of each involved stakeholder. Guides patients in scheduling appointments and laboratory tests according to established processes and policies. Helps patients and care givers with language/literacy barriers to ensure comprehension of information. Identify the particular health care needs of individuals in a community or target area. Navigates access to expertise and experience for the purpose of facilitating knowledge transfer. Performing age related patient assessments and nursing interventions. Practices positive collaboration and converts difficult interactions into productive ones. Provides direct service to internal or external customers.

DEPARTMENT DETAILS

This is a 1.0 position for our survivorship navigator. We are looking for a nurse passionate about survivorship. Some of the job duties would include help run the survivorship clinic and provide education to patients. Work to complete patient survivorship care plans with team. Help to match new patients with cancer mentors and involved in continued education of this group.Work closely with energetic survivorship team. Public speaking at times..

QUALIFICATIONS

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelors Degree in nursing preferred.
