About the Job
This is a part time (20 hours a pay period) day (8:00 am - 4:30 pm) position at ourCrystal Brook location in Park Rapids. This also includes a rotating call.
Scope:
The RN Case Manager is responsible for working with the team to implement and manage the clinical staff and program to ensure the delivery of high quality resident care and services consistent with regulations and established clinical best practices in a caring, and compassionate manner. Performs assessments upon resident admission and creates and monitors a resident care plan. Coordinates admissions and discharges and care for residents of the building. Works closely with the leaders and staff in the building, the nursing staff, residents and families, and the medical community. Provides supervision of Care Attendants and LPNs. Support and reinforce Knute Nelson's mission, vision and beliefs and incorporate them into every customer interaction. Support our safety culture and understand how daily work contributes to the overall safety of customers and staff. Always report safety hazards, follow safety procedures for work area and safe patient handling if applicable. Participate in the stretching exercise program daily and is required to follow correct body mechanics per policy and procedure. Consistently wear name badge while working. Follow and abide by the HIPAA (Health Insurance Portability and Accountability Act) privacy policy as administered by Knute Nelson. Is informed about full range of Knute Nelson services and will advocate and promote internally among employees and externally in the community. Grasp and apply the essence of new information; masters new technical and business knowledge; recognizes own strengths and weaknesses; pursues self-development; seeks feedback from others and opportunities to master new knowledge; models positive attitude with change in the organization. Demonstrate effect time management by prioritizing and executing effectively. Must have flexibility in schedule to fulfill an on-call rotation if required and be available for on-call medical questions and/or emergencies via phone. Must be willing to be on call and flexible to respond to resident and family needs.
