Job Details

The RN Patient Care Coordinator will primarily function in a coordinator role as a resource to staff and physicians involved in the care of Observation status patients. The PCC collaborates with the physician and care team to coordinate the execution of protocols to assist in clinical decision making The role of the Observation Medicine team will be to provide care that expedites the time from observation status to discharge or inpatient admission. PCC will assist with quality improvement projects, coaching to reach department goals and assist with education for staff.

ESSENTIAL JOB FUNCTIONS
  • Supports and models behaviors consistent with the mission, vision and values of Altru Health System.
  • Collaborates with nursing staff on units in care of patient based on
  • Performs pain assessment and takes appropriate action according to standard operating procedure.
  • Advises Manager of significant and unexpected variances.
  • Functions as a resource to staff and physicians regarding clinical and patient care issues.
  • Assists staff in patient care and responds to emergency situations.
  • Anticipates discharge vs admission needs upon transfer to observation status.
  • Provides patient care and instruction, in order to maintain continuity of care on an "as needed" basis. Collaborates with Social Workers when needed.
  • Completes patient rounding to ensure all needs are met. Identifies and takes action on unsatisfactory outcomes, unusual findings, and delays. Coordinates chart checks and conducts follow up calls to patients.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.

KNOWLEDGE & SKILLS
Required:
  • Registered Nurse licensed by the North Dakota Board of Nursing or licensed through the "Compact State Agreement."
  • Must obtain Minnesota license from the Minnesota Board of Nursing within eight (8) weeks from the first day of work. (Cannot practice care in Minnesota until licensed which includes phone calls.)
  • A minimum of three (3) years of clinical experience in an inpatient setting.
  • Demonstrates the ability to effective communicate both verbally and in written format.
  • Successful completion of the functional assessment.
Preferred:
  • Bachelor's Degree in Nursing.
  • Previous supervisory experience.
  • Previous critical care and/or cardiac experience.
  • Demonstrates the ability to use various software programs.

  • Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
  • Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and development and the skills necessary to provide patient/customer care/services.
  • Demonstrates knowledge and understanding of and compliance with:
    • All pertinent safety, health and environmental policies, procedures and guidelines.
    • Working safely, without causing harm or risk to self, others or property.
    • The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader/Manager.
    • The use of all safety equipment and personal protective equipment.
