RN Anticoagulation Clinic Nurse

RC Hospital & Clinics has an opening for an RN Anticoagulation Clinic Nurse /Clinic Care Coordinator.   The anticoagulation clinic nurse is responsible for anticoagulation clinic management and uses a physician directed protocol to determine dosing schedules and changes.  This position is focused on providing education, counseling, and self-management support for individuals requiring chronic disease management.  The Care Coordinator facilitates communication between patients, families and health care provider, functions as patient advocate and navigator. Responsibilities include management and coordination of plan of care in Primary Care office, extending into the community and transitioning patients as they move from acute care to post-acute care services as needed.  Responsibilities also include prescription refills and triage.   

Requirements: RN with Current Minnesota License, 3 years of ambulatory clinic experience with current clinical knowledge as it relates to chronic disease management.  Experience in effectively collaborating and problem-solving with patients, their families and health care team members.  Strong clinical assessment skills, knowledge of community resources and patient education principles.  Proficiency with managing patients on lists classifying patients by risk levels, disease processes and quality measures.  Anticoagulation Therapy Management Certificate preferred.
