The Renal Dialysis Social Worker assists Renal and Inpatient Social Work staff with patient and family plans for transfer; collaborates with community resource persons; ensures patient care plans are reviewed and updated annually; monitors effectiveness of Renal Social Work programs; collaborates with the primary care provider, the Renal Dialysis Medical Director, and the Chronic Kidney Clinic multidisciplinary team; maintains knowledge in reimbursement and other trends in capitation; completes all necessary documentation; completes psychosocial evaluations and ongoing assessments; develops treatment plan and reviews assessment information; provides individual, couple, family or group therapy; makes contact with referral sources and other caregivers; identifies significant changes or high risk situations in patient's condition, and collaborates and functions as a resource for Altru Health System, the community, and the region. Patients cared for include adolescents, adult and geriatric.

Primary Job Duties:
- Demonstrates proficiency in advanced social work skills, including psychosocial assessment and documentation, and intervention in complex situations.
- Assists Renal and Inpatient Social Work staff with patient and family plans for transfer from one setting to another (i.e., hospital to home), as applicable.
- Collaborates with community resource persons to deliver services that promote quality outcomes and continuity of care.
- Ensures patient care plans are reviewed and updated annually or as needed.
- Monitors effectiveness of Renal Social Work programs, policies and services.
- Collaborates with primary care provider, the Renal Dialysis Medical Director, and the Chronic Kidney Clinic multidisciplinary team regarding the client's progress and changing needs.
- Maintains knowledge in reimbursement and other trends in capitation, Medicare, and other third party payors.
- Completes all necessary documentation within appropriate time frames.
- Completes psychosocial evaluations and ongoing assessments on patients, i.e., education, support systems, etc.
- Develops treatment plan and reviews assessment information and treatment plan with each patient.
- Provides individual, couple, family or group therapy for patients and their families using a variety of therapeutic approaches for education, emotional support, and self-help.
- Makes contact with referral sources and other caregivers and assures communication and coordination exists regarding patient care.
- Identifies significant changes or high risk situations in patient's condition and takes appropriate actions as evidenced in reporting, consultations, and documentation.
- Collaborates and functions as a resource for Altru Health System, the community, and region regarding ESRD issues.

KNOWLEDGE & SKILLS REQUIRED :

Required:
• Masters of Social Work (MSW) degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education or has served at least 2 years as a social worker, 1 year of which was in a dialysis unit or transplantation program prior to September 1, 1976, and has established a consultative relationship with a social worker who qualifies under CMS Final ESRD ruling 494.140 (d) (1).
• Licensed by the North Dakota Board of Social Work Examiners or the Minnesota Board of Social Work upon hire.
• Must be dual licensed and until licensed in both states, will be restricted to patients only in state currently licensed in.
• Must possess a theoretical and empirical knowledge base of human behavior development, individual, family and social system abnormalities, working underlying social/economic realities, coping with chronic illness, loss and changes.
• Must have a good knowledge of social work principles and theories and be able to put this understanding into practice through interpersonal skills with both individuals and groups.

SUPERVISION RECEIVED :

Supervision is received from the Manager of Renal Dialysis.
