Guardian Angels Health & Rehabilitation Center is seeking a part-time NAR for day shift.

Position Purpose:
The primary purpose of your job position is to carry out direct resident carein accordance with our established procedures and as may be directed by your supervisors to ensure that the highest standard of nursing services is maintained at all times.

Essential Functions:

Administrative Functions
  • Adhere toSt. Francis Health Services' Mission and live outSt. Francis Health Services' Core Values while performing company business.
  • Maintain confidentiality of company related information at all times.
  • Agree not to disclosure resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Release information only in accordance with established policies and procedures.
  • Use only authorized abbreviations established by the facility when recording information.
  • Take direct appropriate action in emergency situations.
  • Notify supervisor immediately of any observed potential or actual emergencies.
  • Communicate (intra shift) with co-workers, supervisor, and DON as appropriate, regarding resident's progress and your progress with daily resident's care.
  • Maintain a courteous and professional manner in dealing with residents, visitors, medical staff, and fellow employees.
  • Discuss complaints or problems through designated communication channels.

Personal Nursing Care Functions
  • Participate in and receive the nursing report upon reporting to duty.
  • Attend care conferences as assigned.
  • Follow established policies concerning exposure to blood/body fluids.
  • Follow established policies/procedures for performing all cares.
  • Follow written NAR procedure for designated shift.
  • Attend to daily care needs per each resident's plan of care (e.g. bathing, oral care, hair care, nail care, transfers, feeding, ambulation, R.O.M., elimination needs, intake, output, catheter care, dressing, turning and repositioning, collection of specimens, weights and taking vital signs) and document the care each time it is provided.
  • Observe residents regularly and respond promptly to call lights, while showing respect for resident's privacy.
  • Report changes in residents' condition and/or normal routine including nutrition, elimination, ambulation and skin integrity.
  • Understand and follow the Vulnerable Adult and Abuse Policy and reporting procedure.
  • Understand and adhere to Resident Bill of Rights.
  • Provide post-mortem care as instructed.
  • Maintain room order and cleanliness, restock supplies when needed.
  • Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident.
  • Make beds (occupied and unoccupied).
    • Know exact location of all unit equipment and supplies.
    • Seek supervision or assistance when necessary.
    • Perform other duties as assigned by nurse manager, DON, Administrator, or other appropriate person.
Special Nursing Care Functions
  • Provide perineal care.
  • Give tepid sponge baths.
  • Assist with the application of slings, elastic bandages, etc.
  • Perform special treatments as instructed.
    • Turn all medications found in the resident's room/possession (that are not care planned to be there) over to the Director of Nursing Services or Nurse Manager.
  • Watch for and report any change in room temperature, ventilation, lighting, etc.

Staff Development
  • Attend and participate in scheduled training and educational classes that total no less than 12 hours per year, to maintain current certification as a Nursing Assistant.
  • Attend and participate in scheduled onboarding programs and activities.
  • Request and/or accept supervision as needed to develop new skills.

Safety and Sanitation
  • Participate in appropriate in-service training programs prior to performing tasks that involve potential to exposure to blood/body fluids.
  • Keep the nurses' call system within easy reach of the resident.
  • Follow established safety precautions in the performance of all duties.
  • Utilize proper body mechanics.
  • Know exact location of emergency equipment (e.g., alarms and fire extinguishers).
  • Report and/or correct unsafe conditions.
  • Promptly report accidents, errors and unusual incidents to supervisor.
  • Demonstrate and practice correct sanitationtechniques such as hand washing, proper handling of supplies, cleansing and sanitizing equipment per policy procedure.
  • Adhere to basic infection control practices, and use standard precautions.
  • Practice nursing policies and procedures concerning resident restraints and side rails.
  • Know and follow fire plan and tornado alert.
  • Anticipate and act to promote safety for the resident and others.
  • Wear and/or use safety equipment and supplies (e.g. back brace, mechanical lifts, etc.) when lifting or moving residents.
  • Report missing/illegible labels and SDSs to your supervisor.

Equipment and Supply Functions
  • Use only the equipment you have been trained to use.
  • Operate all equipment in a safe manner.
  • Report defective equipment to your supervisor or the nurse in charge.


Ethics

  • Display a consistent interest and enthusiasm for work.
  • Use extra time supporting co-workers, developing rapport with resident, family, etc.
  • Provide considerate and respectful care, preserving the personal dignity, modesty and privacy of the resident and family.

  • Arrive promptly for work and meetings.
  • Exercise responsible judgment for meal and coffee breaks.
  • Give adequate notice to appropriate person when absence is necessary.
  • Maintain respectable appearance in accordance with dress code.
  • Adhere to the Corporate Compliance Program Code of Conduct.
Knowledge and Skills Demonstrate the knowledge and skill necessary to provide care, based on physical, psycho/social, educational, safety and related criteria for adult and geriatric residents.
  • Demonstrate the ability to obtain information and data in terms of resident's needs.
  • Demonstrate an understanding of the range of treatment needed by these residents and ability to provide treatment.
  • Possess communication skills necessary to interpret age specific response to treatment.

Able to operate the following equipment and perform the following procedures:
  • Bathing tubs/showers
  • Mechanical lifts
  • Alarms and other safety interventions/devices
  • Call light systems
  • Charting
  • Transfer belt
  • Specialized chairs, wheelchairs, and positioning equipment
  • Urinary catheters and ostomy bags
  • Dental Care
  • Hearing aids
  • Walkie talkies and/or pagers, if provided by facility

Qualifications
  • Nursing Assistant (NAR) on the MN State Registry in good standing.

Specific Requirements
  • Must be able to read, write, speak and understand the English language while performing company business.
  • Must be a minimum of eighteen (18) years of age
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must display initiative in prioritizing and completing assigned tasks. Understand and demonstrate teamwork.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and at times, the hostile people within the facility.
  • Must be able to relate information concerning a resident's condition.

Physical Requirements:
  • Must be able to move intermittently throughout the work day.
  • Must be able to speak and write the English language in an understandable manner while conducting company business.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to walk, stand and sit for 4-8 hours in an 8-hour shift.
  • Must be able to bend, stoop, squat, twist, reach above shoulders and push/pull frequently.
  • Must be able to lift, carry, or pull 30 pounds. Rarely an NAR may be subjected to lift more than 30 lbs in an emergency situation.
  • Must be able to push 50 lbs.
  • Must be able to lift 31- 60 pounds with assistance.
  • Must follow facility policy and procedures when using a mechanic lift is warranted.
  • Must be able to kneel, crawl and crouch occasionally and climb rarely.
  • Exposed to strain in lifting, moving and supporting residents.
  • Must be able to follow infection control procedures when exposed to infection.
