Seeking CNAs for full time overnights. Hours are 6:30PM to 6AM and working every third weekend.

A Certified Nursing Assistant provides quality resident care in a caring, safe, and efficient manner as specified on the individualized resident care plan according to policies and procedures of the Community Memorial Home.

Reports to: Charge Nurse, DNS

Job Standards:
1. Must be a least 16 year of age and passed an accredited Nursing Assistant Class
2. Must be on the Minnesota State Nurse Aide Registry in good standing
3. Must pass a background check.
4. Must be able to communicate orally or in writing with residents, co-workers, families and public in English.
5. Has desire and ability to care for geriatric or terminally ill residents with respect, caring, and understanding.
6. Must be able to safely perform the essential job functions with or without reasonable accommodation, without posing a direct threat to other employees, residents, or self.
7. Have a pleasant manner and good grooming and hygiene.
8. Must understand the importance of confidentiality and HIPAA as outlined in personnel policies.
9. Must be able to function independently, professionally, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
10. Must be able to work together as a team and work independently.
11. Continuous Quality Improvement--Must actively participate and work to identify innovations, and improve the quality and work outcomes individually and as part of a team.
12. Technology Skills-seeks to learn and utilize appropriate equipment and trends in technology

Physical and Mental Requirements:
1. Must be able to do job according to the following Physical Job Demands
a. Must be able to sit, climb, bend, stoop, squat, crouch and kneel occasionally.
b. Must be able to twist, reach above shoulder height, push/pull wheelchair frequently.
c. Must be able to be on their feet for most of their shift as assigned.
d. Must be able to push, pull, move or lift a minimum of 50 pounds to a minimum height of 4 feet and be able to push, pull, move or carry, such weight a minimum distance of 50 feet.
e. Must be able to read and understand handwriting, regular print, and directions. Must be able to write in a legible manner.
f. Must be able to hear sounds of resident's calls for assistance.
2. Must be able to perform manipulative tasks using items such as safety pins, buttons, snaps, cleaning hearing aids, etc.
3. Must be able to assist residents with transfers and ambulation frequently-Must be able to move in small spaces and between objects frequently.
4. Must have emotional stability to deal effectively with stress created by an ill/dying resident, disoriented/confused residents, residents who may be verbally or physically abusive toward staff, must be able to handle multiple tasks, noises, interruptions and to work cooperatively as part of the health care team while maintaining a pleasant demeanor.

Job Duties:
1. Provides quality resident care as indicated on care plan and resident Kardex:
a. Bathes residents, provides oral hygiene, clean and trims residents' nails
b. Feeds residents using devices and products and encourage independence and dignity
c. Shave residents
d. Dresses residents appropriately
e. Assist residents with continence care per schedule or request
f. Positions residents using appropriate devices
g. Performs range of motion as directed by care plan
h. Assist residents with ambulation and transfers using safety measures
i. Promptly answers call lights and provides care requested
j. Accurately take Vital Signs
k. Measure and records weights, intakes and outputs as directed
l. Responds appropriately to resident behaviors such as confusion or wandering
m. At all times, interacts with residents in a respectful manner, promotes independence by giving choices, communicates caring (verbally and nonverbally), explains procedures before carrying out, etc.
n. Explains all procedures to residents before/during cares
o. Maintains resident-centered conversations while caring for residents
p. Provides personal care for residents without rushing them using a respectful attitude
q. Provides emotional support and redirects residents who are demonstrating inappropriate actions
r. Promotes the highest level of resident ADL functioning

2. Documents and Communicates Changes
a. Reports all changes of condition, skin issues, falls immediately to Charge Nurse
b. Appropriately documents all care and behaviors, completes data flow sheets and uses electronic documentation (PCC) accurately.
c. Receives and shares information, attends and receives resident report prior to providing care

3. Follows facility policies regarding Bill of Rights, Freedom from Abuse, Neglect & Exploitation, etc.
a. Maintains residents' self- esteem, privacy and confidentiality of personal information, adhering to HIPAA guidelines
b. Provide residents with opportunities for independence and choice to wake/bed time, clothing bath type and participation in activities of choice.
c. Safeguard residents' property by labeling, handling with care, and proper storage
d. Interacts with residents, families, and other staff in a respectful manner.
e. Reports any instance of abuse, neglect or mistreatment to charge nurse immediately

4. Provide room order and supplies
a. Make residents' beds, hang resident clothing in closet or place in drawers
b. Keep personal care items within reach but out of sight such as briefs, soaker pads, etc.
c. Keep nightstand, etc. in good order.-watch for food undated, etc.,
d. Keeps hallways clear of objects or keeps objects to one side of the hall for safety.
e. Empty garbage bags every shift or more often as needed.
f. Puts soiled linens and clothing in laundry as needed

5. Follows Safety and Infection Control policies of Care Center
a. Practices good infection control at all times which includes proper hand hygiene including use of hand sanitizing agents, gloves and PPE's.
b. Handles and disposes of linens, soiled clothing or supplies properly
c. Uses proper body mechanics at all times including raising bed
d. Uses appropriate device to lift residents as indicated on Kardex/Care Plan
e. Uses transfer belts for all resident transfers
f. Reports unsafe practices or conditions to supervisor
g. Cleans equipment such as wheelchairs, walkers, basins, hair brushes etc. as directed
h. Responds appropriately to emergency needs in the center (falls, choking, convulsions, fire and weather emergencies, etc.
i. Demonstrates safe use of oxygen, lifts and other equipment
j. Is committed to activities to optimize treatment of infections while reducing the adverse events associated with antibiotic use and to slow down the development and spread of antibiotic resistant infections

6. Professional Performance
a. Attends at least 80% of all CNA meetings.
b. Attends at least 75% of the in-services offered by our facility and all mandatory meetings.
c. Maintain proper dress code including wearing your name tag at all times
d. Must create and maintain a positive attitude and calm environment.
e. Complete Health Care Academy classes on-line annually (12 hours required) as assigned.
f. Follow all Federal/State rules and regulations regarding resident cares.
g. To have dependable attendance with less than 6 absences in a year and be punctual to work. To use TimeStar to document your work time.
h. Report accidents/injuries sustained on the job immediately to the charge nurse.
