RN - Home Care/Hospice

The Registered Nurse performs and documents assessments; identifies the skilled care for the primary certification diagnosis; identifies discharge planning/education needs; works with Case Manager to individualize the patient care pathways on admission; provides patient care; provides patient/patient's family with appropriate education and counseling; identifies significant changes or high risk situations in patient's condition and takes appropriate actions; evaluates and documents patient/patient's caregiver response to treatment, education and progress; reviews patient care pathways with multidisciplinary team, patient and patient's family; conducts on-site supervisory visits; assures completion of all documentation; tracks and assures completion of supervisory visits, recertification/Oasis; screens calls and utilizes telephone triage protocols to triage appropriately in determining needs of caller when on call; demonstrates the ability to adjust own workload; participates in performance improvement monitoring; ensures health history is completed; ensures that intervention or referrals are made promptly; assumes delegated Case Manager, Patient Care Coordinator or primary nurse responsibilities and participates in performance improvement monitoring. Patients cared for include infants, children, adolescents, adults and geriatric.

Required:

• Registered Nurse.

• Licensure required by North Dakota Board of Nursing and Minnesota Board of Nursing.

• One (1) year acute care facility required.

• Successful completion of functional assessment.

• Must possess valid driver's license and have use of personal vehicle to make home visits.

• Must maintain current liability motor vehicle insurance.

Preferred:

• Baccalaureate degree or Home Health Certification.

• Knowledge of case management philosophy, principles and role of Case Manager and visiting nurse.

• One (1) year Home Health experience.

• One (1) year in medical/surgical area of an acute care facility.
