Job Summary:

The DT&H Team Leader provides leadership and coordination to a team of up to ten support staff and up to thirty individuals receiving services by ensuring that the overall needs and desires of the individual receiving services as identified in the Coordinated Service and Support Plan (CSSP) Addendum are attained. Using effective coordination and assistance to the support staff assigned to the team, the DT&H Team Leader will ensure that the staff receives adequate training to support the clients in making informed choices and in providing opportunities for skill acquisition which will lead to greater self-determination. The DT&H Team Leader is responsible for hiring staff and completing performance evaluations.  The Team Leader provides daily direct support to the persons receiving services and maintains a primary caseload of individuals for whom they facilitate meetings and develop their CSSP Addendums.

 

Essential Duties and Responsibilities:

  • Responsible for ensuring the delivery  of quality and individualized services which meet licensing standards and CARF regulations and ensuring supports to meet the expectations as identified in the CSSP Addendums. Responsible for establishing and ensuring compliance with safety procedures and precautions and responsible for monitoring the effective implementation of the standards and procedures required by the Health Consultant in medication administration.
  • Responsible for maintaining adequate staffing within the team to ensure delivery of quality services and for the supervision of the staff assigned to the teams.  Ensure that the staff assigned to the team receives adequate training including direct observation to assess competency to satisfy the requirements specified in the CSSP Addendums.
  • Provide daily direct support to clients, assure protection and promote the dignity and respect for each individual consistent with CSSP Addendums.
  • Responsible to understand and support individual needs and be sensitive to and supportive of unique methods of communication.
  • Coordinate the meeting dates and times for persons on the primary caseload consistent with the CSSP Addendum and ensure critical information is effectively communicated with support staff, other DT&H Team Leaders, county personnel, and residential staff.
  • Be familiar with a variety of person-centered planning processes and facilitation techniques in order to determine and support the planning process that best meets the needs of the individual.
  • Responsible for evaluating the effectiveness of service delivery, methodologies and progress on the individual's outcomes and for communicating with team members to come to an agreement on future approaches.
  • Complete paperwork as required by all licensing/accreditation regulations and agency policies.
  • Responsible for keeping abreast of changes and new techniques in order to provide state-of-the-art services to the individuals assigned to the team.
  • Follow safety procedures and assist in identifying safety needs for self and others.
  • Contribute to the organization's mission by complying with policies and procedures and performing other duties of a similar nature or level.

 

Qualifications/Requirements:

  • Bachelor's Degree in a related field to human services plus one year of full-time work, or;     
  • Associate's Degree in a related field to human services plus two years of full-time work experience providing direct care services to people with disabilities or age 65 and older, or;
  • Diploma in a field related to human services from an accredited postsecondary institution plus three years of full-time work experience providing direct care services to people with disabilities or age 65 and older. or;
  • Four years of full-time work experience providing direct care services to people with disabilities, or age 65 and older, under the supervision of a staff person who meets the qualifications listed above.
  • At least one year of experience working with individuals with developmental disabilities or related conditions, depending on education.
  • Strong supervisory skills.
  • Excellent verbal, written, and interpersonal communication skills.
  • Record keeping abilities.
  • Workplace safety knowledge and familiarity with a variety of person-centered planning processes and facilitation techniques.
  • Must pass both the State of Minnesota Department of Human Services background check and physical exam indicating ability to perform physical demands of the job.
  • Have a valid driver's license, clean driving record and reliable transportation to use for work driving purposes.
  • Strong compassion and understanding of the needs of individuals.
  • Ensure the security and protection of clients at work
