Job Summary

The Panel Specialist contributes to the success of the patient centered health care model of care that originates within primary care clinics by ensuring overall quality patient care. The specialist ensures overall treatment plans are cost efficient, patient plans are understandable for all involved, processes are streamlined for patients, and care is fully supported by the entire care team.

Essential Job Functions

Title and Description

Handles patient appointment reminder duties as they may relate to upcoming preventative and chronic disease needs.

Serves as a patient care liaison by closely interacting with the entire patient care team and appropriate external providers and agencies involved in the care plan.

Reviews patient medical records for accuracy and to ensure they accurately reflect the correct primary care physician/home location.

Completes prior authorization paperwork for medications and follows up on paperwork. Generates, completes and submits other paperwork and fulfills other clerical duties.

Actively participates in the patient referral process by initiating these appointments within the Altru Health System, as well as external providers and agencies, as appropriate. Tracks these appointments, follows-up on them and works to ensure the process is as stream-lined and as cost-effective as possible for the patient.

Monitors care opportunity reports for accuracy and works to resolve any issues/problems in a timely manner.

Assist with Transitional Care Management by working with patients post hospital and post ED visits.

Performs other duties as assigned or needed to meet the needs of the department/organization.

  • Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
  • Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
  • Demonstrates knowledge and understanding of and compliance with:
    • All pertinent safety, health and environmental policies, procedures and guidelines.
    • Working safely, without causing harm or risk to self, others or property.
    • The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader.
    • The use of all safety equipment and personal protective equipment.
