Program Coordinator

Divine House is currently hiring a Program Coordinator to work as well supervise direct support personnel in one of our adult foster car homes in Willmar. This site will be opening with 1 adult female and be licensed up to 4 residents.

Qualifications:
Must have the ability to effectively communicate and work under the direction of a variety of other professionals. A Program Coordinator must possess problem solving skills and have a strong desire to help others.

PRINCIPLE RESPONSIBILITIES:
• To serve as a vital link in the communication and coordination of services with the person, their expanded support team, and administration to ensure continuity of care.

• To provide supervision of direct support staff to ensure the health, safety and protection of rights of each person you oversee and implementation of the responsibilities assigned to the License Holder in the Coordinated Service and Support Plan and support plan addendum

• Advocate and provide support to promote the needs, interests, and preferences of the person(s) served

A Human Service Technician (HST) must possess basic problem solving skills, have a strong desire to help others, and the desire to improve the quality of life of the individuals they are working with.

An HST is responsible for following/implementing an individual’s plans, assist in activities of daily needs such as bathing, dressing, toileting, etc., assisting in instrumental activities of daily living such as shopping, food prep, laundry, etc., and medication administration.

An HST must have the ability to effectively communicate orally and in writing within the house as well as within the company departments and document in the necessary records.

HSTs must also perform daily duties and responsibilities as assigned, such as but not limited to: housekeeping duties, household errands, appointments, shopping, and leisure activities. Each house can vary in the needs of each individual served.
