Position Summary

• Responsible for providing excellent medical care to palliative, hospice, and primary care patients

• Collaborates with multi-disciplinary teams comprised of physicians, NPs, PAs, RNs, LPNs, Chaplains, Social Workers, and Health Assistants/CNAs, and administration to provide patient care and family/caregiver support

• Requires a high level of communication that is clear, concise, patient-centered; team education/guidance is vital to our patient care and mission accomplishment

• Significant travel is required; vehicle will be provided or mileage reimbursed

• Care will be delivered in compliance with all pertinent regulations including the hospice conditions of participation, corporate compliance, quality standards, licensure regulations and will reflect the national standards of care for hospice services

 

Essential Functions

• Provides pain, disease, and symptom management for patients

• Determines continued patient eligibility through face-to-face visits, documentation review and clinical interactions

• Consults with nursing staff regarding medical care of patients

• Collaborates on education/training of patient care staff, referral sources, medical institutions, and the community/public

 

Basic Qualifications

• Doctorate in Nursing Practice from accredited institution of higher learning

• Six years of progressive clinical/medical practice based on current research, trends, and best practices

• Solid understanding of palliative and hospice care as it relates to community healthcare organizations

• Solid understanding of palliative and hospice needs from profit, non-profit, federal healthcare sectors, including needs of patients and family/caregivers wherever the patient calls home (Including assisted living, and varieties of nursing facilities)

 

Desired Qualifications

• Ten years progressive clinical/medical practice based on cutting edge medical care delivery

• Four years of progressive palliative and hospice experience

• Educator at accredited institution of higher education or published healthcare author, or state-/national-level speaker on healthcare topic such as palliative care, hospice care, primary care, long-term health concerns, or similar

• Significant background in pain and symptom management

• Significant experience in aging/geriatric population health management

 

Certifications, Licenses, Registrations Required

• DEA

• Licensure in North Dakota and Minnesota with ability obtain licensure in other states as required

• Current drivers license and auto insurance
