Under the direct supervision of a licensed nurse, the Trained Medication Aide is responsible for administering medications in accordance with established protocols of Rice Care Center.
JOB RELATIONSHIPS: A. REPORTS TO: Director of Nursing, Charge Nurse, LPN B. SUPERVISES: No one C. INTERRELATIONSHIPS WITH: All departments within the facility, families, physicians, and external customers.
HOURS OF WORK: Rotating shifts. Variable hours dependent on census/case load.
DRESS CODE: Name badge, uniform top & pants. Shoes with socks. (See dress code policy)
ESSENTIAL REQUIREMENTS OF WORK (Minimum qualifications necessary to function at full productivity). A. EDUCATION AND TRAINING: Must be certified as a Registered Nursing Assistant and be in "good

standing" on the Minnesota Nursing Assistant Registry. Must be at least 18 years of age. Unlicensed nursing personnel must have completed a medication administration training program which is offered through a Minnesota postsecondary educational institution. See regulation from Minnesota Administrative Rules 4658.1360 ADMINISTRATION OF MEDICATIONS BY UNLICENSED PERSONNEL.

B. EXPERIENCE: Previous experience as a Nursing Assistant or Trained Medication Aide preferred.

Has satisfactorily completed the training program required. A minimum of two (2) years of experience as a Nursing Assistant is required for the position. Must enjoy working with the geriatric population.

C. JOB KNOWLEDGE (Specific): Must have a working knowledge of medication administration and

must understand the established limits within the scope of practice relating to the TMA job responsibilities. Ability to communicate orally and in writing. Ability to establish effective working relationships with residents, co-workers and families. A specific understanding of the cognitive, physical and psychological changes experienced in the geriatric population. An understanding of the range of treatment and medications needed by residents of all ages in relation to the developmental aging process, with a focus of attention towards the geriatric population. Successful completion of the TMA coursework and proof of on-going continuing education.

MENTAL DEMANDS OF THE JOB: A. JUDGEMENT AND INITIATIVE: Works under the direction of the RN/LPN, But must be able organize efficiently while providing individualized resident care. Receives direction from the licensed nurse. B. INDEPENDENT ACTION: Work involves a variety of duties that are performed according to facility procedure and involves self-motivation to complete tasks and provide assistance to others. Within parameters of delegated activity, must perform with limited supervision. C. EFFECT OF ERROR: Potential for resident care related error. Error unacceptable D. CONTACT WITH OTHERS: Contact with other nursing personnel, residents,

families, and other departments within the facility, and also our external customers.
PRINCIPAL JOB RESPONSIBILITIES, TASKS, AND AUTHORITIES: A. RESPONSIBILITY: Medication Administration PRIORITY: A % OF TIME 80% DISCRETION: D TASKS: 1. The TMA gives medications using only the following routes of administration: oral, including sublingual, rectal and topical. The TMA will not administer IV's, sub-q injections, vaginal suppositories, urethral medications, or give any form of intramuscular medication. 2. Medication Administration is not delegated to a Trained Medication Aide

when the following conditions exist: a) resident is comatose, semi-comatose, or combative. b) dosage or administration depends on physiological measurements; e.g. blood

pressure, temperature, pulse, respiration and prothrombin time (RN needs to make assessment)

c) Resident is undergoing detoxification d) resident is unstable e) calculation of dosage f) conversion of measuring scales g) sterile techniques h) resident medication regimen has not been reviewed by a physician I) can not score tablets 3. TMA's are NOT allowed to give Scheduled II narcotics, routine or PRN. Routine scheduled III

or IV are allowed. All PRN Medications MUST be given under the direct supervision of a nurse.

4. The TMA is not allowed to accept/take verbal or telephone orders from a physician. The TMA is not allowed to transcribe physician orders. 5. The TMA can not administer naso-gastric or gastrostomy tube medications unless this is a

procedure delegated to the TMA by the supervising licensed nurse. If this procedure is delegated by the nurse, the TMA must be taught those procedures by the nurse, and education taught by the nurse will be documented in the TMA's employee education file.

6. The TMA is responsible to research any medications that are unfamiliar, by looking medications up in the reference manual provided on all med carts and at the nurse's desk.

7. The TMA is not allowed to destroy controlled substances unless witnessed by the licensed nurse. He/she may witness nurse destroying controlled substances.

8. The TMA is not allowed to prepare administration cards/med sheets. 9. The TMA must document medications consistently and thoroughly on the E-MAR. 10. The TMA is allowed to order drugs by faxing the pharmacy or calling the pharmacy. 11. The TMA is responsible for reporting changes in the resident's condition

to the licensed nurse. Is responsible to and notifies supervisor of emergency situations.

12. Observes residents response to medications 13. Assures that resident safely took medication given to him/her. 14. Reports any concern of reaction to medication given to resident to licensed nurse immediately. 15. The TMA is allowed to administer nebulizers or inhalers with education/training. 16. TMA may document after receiving training from Medical Records personnel. 17. Computer charting only after Charge Nurse & TMA discuss information that needs to be charted. B. RESPONSIBILITY: Other Direct Resident Care Activities PRIORITY: A % OF TIME: 10% DISCRETION: D TASKS: 1. Gives direct care to residents as needed. 2. Reports changes in resident condition to licensed nurse immediately. 3. Assists with answering call-lights. 4. Assists with feeding residents at mealtimes. 5. Follows up on observations reported by NA/R's. Reports observations to

licensed nurse on duty. 6. Listens to nursing report prior to starting shift, and reports at end of shift. 7. Receives report from NA'R's and reports findings to Charge Nurse. 8. Observes privacy and dignity issues as they relate to resident care. 9. Check with Charge Nurse regarding resident charting. C. RESPONSIBILITY: Other Activities Related to Resident Care PRIORITY: A % OF TIME: 10% DISCRETION: B TASKS: 1. Develops and maintains interpersonal relationships with residents, families, and visitors. 2. Keeps resident information confidential and respects resident's rights. 3. Assists in maintaining a clean, safe environment for residents and co-workers. 4. Is familiar with safety and emergency disaster plans. 5. Participates in inservice programs for self-enrichment and seeks further growth with educational opportunities. Attends staff & department meetings.

6. Reports (and where appropriate, records) any changes observed in condition or behavior of resident. Communicates any resident complaints of pain to resident nurse, charge nurse. Participates in resident care conferences as requested. D. RESPONSIBILITY: Continuous Quality Improvement TASKS: 1. Demonstrates understanding of the CQI philosophy 2. Participates in CQI training activities or CQI teams if appropriate. 3. Demonstrates sensitivity to customers and their needs. 4. Interacts appropriately with internal customers and their needs. 5. Interacts appropriately with internal customers, i.e. coworkers within the department, staff

across departments. 6. Role models positive behaviors. 7. Actively involved in Performance Improvement process at RCC within Nursing Dept. structure. E. RESPONSIBILITY: Resident Rights TASKS: 1. Acts as an advocate for Resident Bill of Rights 2. Assures that all residents, families and staff are aware of resident rights 3. Assists in the identification of vulnerable adult issues 4. Respects resident property 5. Maintains dignity and privacy of residents by knocking before entering resident room, addressing them in a respectful manner, discussing their status in a private manner, treating them in a courteous and polite manner.

F. RESPONSIBILITY: Service Excellence TASKS: Demonstrates an ongoing commitment to the Service Excellence philosophy by adhering to

G. RESPONSIBILITY: Safety - Maintain and promote a safe environment for all patients, visitors and staff. TASKS: 1. Consistently follow all policies, practices and work rules. 2. Do not use shortcuts or work-arounds that may reduce safety or increase risk. 3. Stay alert, act responsibly and use common sense to reduce risks. 4. Report actual events and good catches as soon as possible. 5. Create a safe environment by eliminating hazards and identifying and reporting unsafe

systems. 6. Complete all mandatory safety education, attend safety sessions, review and understand
