Job Summary

The Regional Transfer Coordinator works with regional facilities to assess patients' need for appropriate transfer to hospital care within Altru Health System provider. Receives and documents all patient information, assists all parties involved in the transfer, provides a seamless well-coordinated transfer/admit process, establishes effective communication processes, develops working relations with key regional access sources, and utilizes clinical skills in triaging patients.

Essential Job Functions

Utilizes clinical skills in triaging patients for appropriate services and consults specialists if needed to provide best possible care for the patients. Arranges provider to provider consults for discussion of patient condition prior to acceptance, if needed.

Participates in the marketing activities for the promotion and development for regional access services.

Works with regional facilities to assess patients' need for appropriate transfer to hospital care within Altru Health System (AHS) provider.

Develops working relationships with key regional access sources including AHS branch clinics. Establishes good working relationships with regional access sources and department staff including physicians, nursing, and scheduling.

Monitors services by evaluating, initiating and participating in restructuring of current and new programs and services.

Assists all parties involved with the transfer of urgent/same day admissions and coordinates nursing unit and bed arrangements according to standing operating procedure. Provides a seamless, well-coordinated transfer/admit process and a centralized point of access.

Receives and documents all patient information, and communicates to accepting provider to assure continuity of care is maintained. Establishes effective communication processes with regional referral providers to keep providers up-to-date with patient status by facilitating communication between the accepting and receiving providers.

Performs other duties as assigned or needed to meet the needs of the department/organization.
  • Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
  • Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
  • Demonstrates knowledge and understanding of and compliance with:
    • All pertinent safety, health and environmental policies, procedures and guidelines.
    • Working safely, without causing harm or risk to self, others or property.
    • The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader
    • The use of all safety equipment and personal protective equipment.

License, Certification & Registration

Registered Nurse - North Dakota Board of Nursing (NDBON) or Compact State Agreement; Prior to Start Date

Registered Nurse - Minnesota Board of Nursing; Prior to Start Date

Basic Life Support (BLS) - American Heart Assoc. or American Red Cross CPR for Prof. Rescue; Within 2 Months of Start Date

Advanced Cardiac Life Support (ACLS) - American Heart Assoc. or American Red Cross CPR for Prof. Rescue; Within 2 Months of Start Date

Pediatric Advanced Life Support (PALS) - American Heart Assoc. or American Red Cross CPR for Prof. Rescue; Within 2 Months of Start Date

Education & Experience

A bachelor's degree in a related field is preferred
